Efeito de um programa de fortalecimento muscular na espasticidade do quadríceps femoral em sequelados de acidente vascular encefálico

Abstract

The aim of this study is to analyze the effect of a program of muscle strengthening on spasticity and strength of the quadriceps in patients with stroke sequelae. The sample was composed of 18 individuals with sequelae of stroke, being 12 in the experimental group (EG) and 6 in the Control Group (CG). Strength and muscle tone were measured using suitable electronic instrumentation and digital polygraph BioMed ® software. The GE program activities carried out with five minutes of heating on the exercise bike, three sets of passive stretching, twenty seconds each, the quadriceps femoris, and ending with strengthening the quadriceps muscle, using a Bonnet chair. The building program consisted of ten sessions over a period of three weeks, using a resistance of 50% of one repetition maximum (RM) in the first six sessions and 80% in the last four. This program is tailored to individuals affected by stroke, being composed of three sets of ten repetitions, spastic limb, followed by a series of ten repetitions in the limb sound, and ending with another series of ten repetitions again spastic limb. At the end of each series was maintained isometric contraction of 10/2, and between sets, a rest interval of one minute. The GC was evaluated in the same periods of the experimental group, but without intervention. The data were analyzed using SPSS 16.0 software, the mean, standard deviation, minimum, maximum, Shapiro-Wilk, Levene's test, paired Student t test and Wilcoxon. The strength showed no statistically significant difference. The same happened to the value of Root Mean Square (RMS) of the electromyogram (EMG) measured at maximal isometric contraction in both the vastus medialis and the vastus lateralis in both groups. The RMS value of EMG, in the EG, at rest showed statistically significant differences, between before and after strength training for both the vastus medialis (p = 0.025), and the vastus lateralis (p = 0.025). However, the CG at rest, showed no statistically significant difference either in the vastus medialis or the vastus lateralis. So, we can observe that the strengthening of the muscles affected by stroke, did not increase spasticity.

O objetivo deste trabalho é analisar o efeito de um programa de fortalecimento muscular sobre a espasticidade e a força do quadríceps femoral em pacientes com sequelas de AVE. A amostra foi formada por 18 pessoas sequeladas de AVE, sendo 12 no Grupo Experimental (GE) e 6 no Grupo Controle (GC). Força e tônus muscular foram mensurados, usando instrumentação eletrônica adequada e o software polígrafo digital BioMed®. O GE realizou programa de atividades composto por cinco minutos de aquecimento na bicicleta ergométrica, três séries de alongamento passivo, de vinte segundos cada, no quadríceps femoral, e finalizando com fortalecimento muscular do quadríceps, utilizando-se a cadeira de Bonnet. O programa de fortalecimento foi composto por dez sessões, por um período de três semanas, utilizando-se uma resistência de 50% de uma repetição máxima (RM) nas seis primeiras sessões e 80% nas quatro últimas. Este programa é adaptado a indivíduos acometidos por AVE, sendo composto por três séries de dez repetições, no membro espástico, seguida de uma série de dez repetições no membro sadio, e finalizando com mais uma série de dez repetições novamente no membro espástico. Ao final de cada série, foi mantida uma contração isométrica de dez segundos, e entre as séries, um intervalo de descanço de um minuto. O GC foi avaliado nos mesmos períodos do grupo experimental, porém sem intervenção. Os dados foram analisados usando o software SPSS 16.0, sendo calculada média, desvio padrão, mínima , máxima, Shapiro-Wilk, teste de Levene, teste t de Student pareado e Wilcoxon. A força não apresentou diferença estatística significativa. O mesmo aconteceu com o valor Root Mean Square (RMS) do Eletromiograma (EMG) medido em contração isométrica máxima. O valor RMS do EMG, no GE, durante o repouso, apresentou diferença estatística significativa, tanto para o músculo vasto medial (p=0,025), como o vasto lateral (p=0,025), o que não aconteceu com GC. Observa-se que o fortalecimento da musculatura acometida por AVE, não produziu aumento da espasticidade.
