Abstract

O sangue é um fluído vital para o equilíbrio homeostático do organismo, constituído maioritariamente por plasma e elementos figurados. Os eritrócitos, em maior número, são responsáveis pelo transporte de gases e equilíbrio do pH, produzidos por intermédio da hormona eritropoietina, estimulante da eritropoiese. Este processo acontece essencialmente durante o sono e em resposta a hipoxia. Assim, um sono de má qualidade impede a normal produção de eritropoietina, dificultando a eritropoiese e consequentemente a diminuição da produção de eritrócitos e hemoglobina, promovendo o desenvolvimento de patologias. Este problema toma maiores proporções em indivíduos que possuam trabalhos por turnos, como é o caso de bombeiros. O presente trabalho permitiu determinar o perfil hematológico e avaliar a qualidade de sono de 239 operacionais de 14 Corporações de Bombeiros Voluntários do distrito de Bragança, de forma a verificar a potencial implicação da má qualidade do sono no hemograma e na saúde deste grupo social. A colheita de dados decorreu entre 2021 (julho) e 2023 (agosto), pela aplicação do questionário sociodemográfico e do Índice de Qualidade de Sono de Pittsburgh. Foi ainda recolhida uma amostra de sangue total para determinação do hemograma. Os participantes assumiram o consentimento informado e toda a investigação respeitou a Declaração de Helsínquia e a Convenção de Oviedo, tendo sido previamente aprovado pela Comissão de Ética da Universidade do Porto. A análise estatística foi feita com recurso aos testes paramétricos para testar a normalidade da distribuição e os testes não paramétricos para comparação e correlação entre variáveis. Relativamente aos resultados hematológicos obtidos, verificou-se que a contagem de eritrócitos, a concentração de hemoglobina e a percentagem do hematócrito possuíam valores normais em relação aos valores de referência. Considerando os resultados da qualidade de sono, verificou-se que 45% dos bombeiros apresentaram má qualidade de sono, o que vai de encontro a outros trabalhos descritos na literatura no qual obtiveram resultados semelhantes. Através da análise estatística, observou-se uma correlação estatisticamente significativa entre a qualidade de sono e a produção de eritrócitos (r=-0,157; p=0,026). Embora o perfil hematológico dos bombeiros que participaram no estudo, ainda se mantenha na generalidade, no intervalo de referência, é já visível uma tendência para a aproximação de níveis inferiores do intervalo Conclui-se que é crucial, prosseguir estudos para melhor entender a relação entre a qualidade de sono e o perfil hematológico, assim como explorar os mecanismos de ação subjacentes.

Blood is a vital fluid for the body's homeostatic balance, consisting mainly of plasma and formed elements. Erythrocytes, in greater number, are responsible for transporting gases and balancing pH, produced through the hormone erythropoietin, which stimulates erythropoiesis. This process essentially occurs during sleep and in response to hypoxia. Thus, poor quality of sleep prevents the normal production of erythropoietin, hindering erythropoiesis and consequently reducing the production of erythrocytes and hemoglobin, promoting the development of pathologies. This problem takes on greater proportions in individuals who work shifts, such as firefighters. The present study made it possible to determine the hematological profile and evaluate the sleep quality of 239 operatives from 14 Volunteer Fire Brigades in the district of Bragança, in order to verify the potential implication of poor sleep quality on the blood count and the health of this social group. Data collection took place between 2021 (July) and 2023 (August), using the sociodemographic questionnaire and the Pittsburgh Sleep Quality Index. A whole blood sample was also collected to determine the blood count. Participants gave informed consent and the entire investigation respected the Declaration of Helsinki and the Oviedo Convention, having been previously approved by the Ethics Committee of the University of Porto. Statistical analysis was carried out using parametric tests to test the normality of the distribution and non-parametric tests for comparison and correlation between variables. Regarding the hematological results obtained, it was found that the erythrocyte count, hemoglobin concentration and hematocrit percentage had normal values in relation to reference values. Considering the results of sleep quality, it was found that 45% of firefighters had poor sleep quality, which is in line with other studies described in the literature in which they obtained similar results. Through statistical analysis, a statistically significant correlation was observed between sleep quality and erythrocyte production (r=-0.157; p=0.026). Although the hematological profile of the firefighters who participated in the study still remains generally within the reference range, a tendency towards lower levels of the range is already visible. It is concluded that it is crucial to continue studies to better understand the relationship between sleep quality and hematological profile, as well as explore the underlying mechanisms of action.
