A anemia é uma condição clínica caraterizada pela redução no número de eritrócitos, que resulta na diminuição do hematócrito e da concentração de hemoglobina. É considerada uma síndrome, uma vez que apresenta um conjunto de sintomas e evidências físicas que não remetem diretamente para a causa. A resposta compensatória do organismo a esta condição é classificada como uma anemia regenerativa. A regeneração indica que o organismo foi capaz de responder e combater de forma apropriada a anemia, habitualmente através da medula óssea, pela intensificação da produção e libertação para a corrente sanguínea de eritrócitos jovens, os reticulócitos, que em comparação com um eritrócito maduro são maiores e com menos hemoglobina do que este. Para a classificação de uma anemia como regenerativa ou não regenerativa devem ser considerados vários aspetos do eritrograma desde os índices eritrocitários, à observação da morfologia celular no esfregaço sanguíneo bem como a contagem absoluta de reticulócitos, este considerado atualmente como o indicador mais preciso de regeneração. A avaliação dos índices eritrocitários como o volume corpuscular médio (VCM) e a concentração de hemoglobina corpuscular média (CHCM) também tem mostrado a sua relação com a resposta regenerativa, sendo que a macrocitose e a hipocromasia são associadas por vários autores à regeneração medular. Foi então objetivo deste estudo verificar se, para o cão e para o gato, o VCM e o CHCM e a avaliação da morfologia eritrocitária estão em concordância na identificação de uma anemia como regenerativa ou não regenerativa. Para o gato, também foi objetivo perceber qual dos métodos é o mais fiável na classificação da regeneração medular em comparação com o método de referência, a contagem absoluta de reticulócitos. Um Total de 150 casos de anemias em cães e 145 em gatos foram classificados prospectivamente por dois métodos, índices eritrocitários e morfologia de eritrocitária e no caso dos gatos posteriormente comparados com um terceiro método, a contagem absoluta de reticulócitos. Da análise dos resultados, concluiu-se que, para ambas as espécies, os dois métodos em estudo não são concordantes quanto à avaliação da regeneração medular. Além disso, por comparação com a contagem absoluta de reticulócitos, no gato, percebeu-se que a avaliação microscópica da morfologia celular em esfregaço sanguíneo é um método mais fiável para classificar uma anemia como regenerativa. Nos cães, os resultados foram inconclusivos. Apesar do avanço tecnológico, os analisadores hematológicos apresentam limitações no que diz respeito ao reconhecimento de células anormais e, por isso, vários investigadores consideram importante recorrer a outros métodos. A análise dos dados do hemograma conjugada com a avaliação da morfologia celular em esfregaço sanguíneo é muito importante e auxilia numa caracterização mais correta da anemia.
Anemia is a clinical condition characterized by the reduction in the number of erythrocytes, which results in a decreases hematocrit and hemoglobin content. Anemia is considered a syndrome as it presents a group of symptoms and physical evidence that are not directly relatedto the disease. Regenerative anemia is classified as the body’s compensatory response to this condition. Regeneration indicates that the body is able to respond appropriately to the anemia, usually through the bone marrow intensified production and release of immature erythrocytes in bloodstream, the reticulocytes, that are bigger and have less hemoglobin content. To classify an anemia as regenerative or non-regenerative several aspects must be considered,including erythrocyte index values, the morphological observation of blood smears and the absolute count of reticulocytes, the last one considered the most accurate indicator of regeneration. Evaluation of mean corpuscular volume (MCV) and mean corpuscular hemoglobin concentration (MCHC) have also shown their relationship with the regenerative response, with macrocytosis and hypocromasia being associated with regeneration by several authors. The purpose of this work was to determine whether, for dogs and cats, MCV and MCHC values and erythrocyte morphology are in agreement identifying an anemia as regenerative or non-regenerative, and to find the most reliable method in classifying the regenerative response in cats, comparing to the reference method. A total of 150 cases of anemia in dogs and 145 in cats were prospectively classified by two methods, erythrocyte indices and erythrocyte morphology and subsequently compared with a third method, the absolute reticulocyte count, for cats. The analysis of results revealed, for both species, that the two methods used in the study were not concurrent regarding the assessment of medullar regeneration. Furthermore, when compared to the absolute reticulocyte count, microscopic evaluation of cell morphology in a blood smear proved to be a more reliable method for categorizing an anemia as regenerative in cats. In dogs, the results were inconclusive. Despite the tremendous technological advances, hematological analyzers have limitations when it comes to recognize abnormal cells and, therefore, several researchers consider that it is important to resort to other methods. The analysis of blood count d data combined with the assessment of cell morphology in blood smears is very important and helps in a more correct characterization of anemia.
