Classificação automática de termogramas do pé diabético usando técnicas de Machine Learning

Abstract

Diabetic foot is one of the main complications observed in diabetic patients, it is associated with the development of foot ulcers and can lead to amputation. As the risk of ulceration is directly linked to an increase of the temperature in the plantar region, several studies use thermography as a method for automatic identification of problems in diabetic foot. As the distribution of plantar temperature of diabetic patients do not follow a specific pattern, it is difficult to measure temperature changes and, therefore, there is an interest in the development of methods that allow the detection of these abnormal changes. To aid their decisions and help prevent mistakes, the resort to computer assisted diagnostic systems using artificial intelligence techniques is gradually increasing, since these systems can help health professionals to improve the accuracy of the exams, increase the consistency in the interpretation of the images, assist in the prognostic evaluation and support the therapeutic decision making. The objective of this work is to develop a methodology that uses thermograms of the feet of both diabetic and healthy individuals to classify and analyse the thermal changes diversity in the plantar region. For this, two classification models of thermograms for the feet of diabetic and healthy individuals are proposed and compared. In order to detect and classify abnormal changes in the plantar temperature, machine learning algorithms are used in both models. The first model, Model 1, classifies the foot thermograms into four classes: healthy and three categories for diabetics. The second model, Model 2, has two stages: in the first stage, the foot is classified as belonging to a diabetic or healthy individual, while, in the second stage, a classification refinement is done, classifying diabetic foot in 3 classes of progressive severity. The illustrated results show that both proposed models have a satisfactory performance, however, when compared Model 2 outperforms Model 1 and allows a better classification performance concerning the healthy category and the first class of diabetic individuals. These results demonstrate that the proposed methodology can be a tool to aid medical diagnosis.
