Caracterización molecular de Escherichia coli bacteriémicos pertenecientes a los serogrupos O1 e O2

Abstract

[ESP] En los últimos años se ha observado un aumento de bacteriemias causadas por E. coli patógenos extraintestinales (ExPEC), E. coli uropatogénicos (UPEC) y E. coli patógenos aviares (APEC). Entre los serogrupos más prevalentes a nivel global se encuentran los serogrupos O1 y O2. Nuestro objetivo es caracterizar las cepas pertenecientes a estos serogrupos, buscando entender las razones de su éxito. Determinar las características de estas cepas es vital para diseñar vacunas adaptadas a la epidemiología actual de nuestra área sanitaria. Para el estudio y caracterización de las 53 cepas de E. coli causantes de bacteriemias en un hospital lucense (HULA) durante los años 2020 a 2022 utilizamos métodos fenotípicos (serotipado, determinación de CMIs) y moleculares. Determinamos serotipos, grupos filogenéticos y genes de virulencia para establecer los estatus ExPEC, UPEC y APEC. La colección incluye 20 cepas O1, 33 O2, de las cuales solo 6 resultaron ser multirresistentes (MDR). La caracterización molecular muestra que el grupo filogenético más abundante es el B2 (42 cepas). Las cepas del presente estudio mostraron una amplia colección de factores de virulencia. En base a sus genes de virulencia, 46 cepas se calificaron como ExPEC, 48 UPEC y 31 APEC. Además, se detectaron 24 aislados pertenecientes a la secuencia tipo 95. Los resultados confirman que un porcentaje significativo de cepas causantes de septicemias en la población lucense son positivas para ExPEC, UPEC y APEC. Asimismo, se reafirma la idea de que el éxito de los serogrupos O1 y O2 no se debe a su resistencia a los antibióticos si no al elevado número de factores de virulencia.
