Abstract

La infección persistente por subtipos de alto riesgo oncogénico del Virus Papiloma Humano es el principal factor causal del cáncer de cérvix, siendo esta neoplasia, la primera causa de mortalidad por cáncer en mujeres menores de 45 años en países subdesarrollados. Pese a que la infección por este virus presenta una alta prevalencia, el riesgo de desarrollar cáncer de cérvix es relativamente bajo, pues la mayoría de las infecciones son transitorias y son eliminadas naturalmente por el sistema inmune. Sin embargo, algunas de las mujeres que entran en contacto con el virus desarrollan una infección persistente, la cual se asocia a mayor riesgo de transformación neoplásica. Si bien las indicaciones de manejo de pacientes con anomalías histológicas y citológicas cervicales son similares en diversas guías clínicas, el manejo clínico y terapéutico de mujeres con infección viral persistente sin anomalía citológica asociada no está bien establecido. En este trabajo, se realiza una revisión bibliográfica sobre la infección persistente por Virus Papiloma Humano en ausencia de anomalía citológica. Así, se revisan factores de riesgo, historia natural, epidemiología, riesgo de transformación neoplásica, manejo clínico, influencia de la vacunación, tratamientos actuales y líneas de investigación en desarrollo.
