Abstract

El estrés oxidativo es un mecanismos implicado en disfunción endotelial y alteraciones vasomotoras relacionadas con hipertensión arterial (HTA). La cronoterapia a dosis nocturnas ha demostrado mejorar cifras nocturnas de presión arterial (PA) y reducir morbimortalidad. Sin embargo, el papel del estrés oxidativo en las anormalidades del perfil circadiano de PA, y la relación entre cronoterapia, niveles de estrés oxidativo e índices de PA es aún poco conocida. Nuestro objetivo fue evaluar las diferencias en los niveles de algunos marcadores redox entre pacientes hipertensos con un perfil dipper y no-dipper de PA, estudiar las correlaciones entre dichos marcadores y los índices de PA, y evaluar el impacto de la cronoterapia a dosis nocturnas sobre dichos índices y niveles de marcadores redox. Esta tesis doctoral presenta un diseño observacional de corte transversal en el que hemos incluido pacientes hipertensos mayores de 18 años. Las variables recogidas incluyeron índices de monitorización ambulatoria de la PA durante 24 h. Los marcadores redox evaluados fueron las substancias reactivas al ácido tiobarbitúrico (TBARS), tioles reducidos, vitamina A, vitamina E y metales (cobre y zinc). Los niveles de TBARS y ratio cobre/zinc se asociaron a un riesgo aumentado de un perfil no-dipper de PA. Los niveles de cobre, tioles reducidos y vitaminas liposolubles mostraron correlación con algunos índices nocturnos de PA. Los pacientes hipertensos que usaron bloqueadores del SRAA antes de dormir presentaron niveles más bajos de TBARS que el grupo comparación y correlación entre los niveles de tioles y la profundidad nocturna de la PA diastólica.
