Abstract

Introducción: El objetivo de este estudio consistió en analizar la subestimación del carcinoma ductal invasivo (CDI), en pacientes con diagnóstico preoperatorio de carcinoma ductal in situ (CDIS) e identificar los factores asociados con el riesgo de infradiagnóstico. Métodos: Se estudiaron 78 pacientes con diagnóstico de CDIS tratadas mediante cirugía conservadora de mama (BCS) o mediante mastectomía. Se calcularon los porcentajes combinados y específicos del estudio para las subestimaciones de CDIS, y se investigó la asociación entre cada variable preoperatoria y el IBC. Resultados: El porcentaje global de subestimación IBC fue del 17,94%. La variable preoperatoria que mostró asociación significativa con una mayor subestimación incluye las lesiones con un tamaño inicial superior a 2 cm en comparación con las lesiones iguales o menores a 2 cm. El tipo de biopsia realizada, el grado histológico, la presencia de comedo-necrosis y el estado de los receptores de estrógenos, no se asociaron significativamente con la predicción de la enfermedad invasiva. Conclusiones: El riesgo de infradiagnóstico de IBC en pacientes diagnosticadas inicialmente con CDIS es considerable. Para las pacientes con diagnóstico preoperatorio de CDIS, el tamaño, el tipo de biopsia realizada o la presencia de masas mamográficas, implican un mayor riesgo de carcinoma invasivo. Es importante conocer los factores preoperatorios asociados a la subestimación del IBC para poder seleccionar los pacientes de alto riesgo y planificar un tratamiento adecuado
