Bruxismo: abordagens terapêuticas contemporâneas

Abstract

O bruxismo é definido como uma atividade repetitiva dos músculos mastigatórios caracterizada pelo apertamento ou ranger dos dentes. Tem duas manifestações distintas, bruxismo do sono são atividades musculares mastigatórias que ocorrem durante o sono (rítmico\não-rítmico) e bruxismo de vigília, caracterizado como apertamento repe dos dentes quando acordado. A literatura elucida vários tipos de goteiras oclusais, aplicação de toxina botulínica, fisioterapia com alongamentos, exercícios e massagens nos músculos da mastigação, tratamento cognitivo comportamental com Biofeedback e mudança de hábito, tratamento farmacológico e um dispositivo intra-auricular. Diante das opções de tratamento este trabalho visa fazer uma revisão sobre as abordagens terapêuticas clássicas e tratamentos contemporâneos para bruxismo, suas indicações, contra-indicações, vantagens e desvantagens. Após a revisão da literatura dos últimos 10 anos, sugere-se aos Médicos Dentistas que sigam a abordagem múltipla: goteiras, aconselhamento e psicologia. Há necessidade de mais ensaios clínicos, baseados em amostras representativas, a longo prazo, para avaliar a efetividade e a segurança dos tratamentos propostos para o bruxismo.
