Abstract

Os bisfosfonatos são um grupo de medicamentos utilizados no tratamento de doenças malignas metastáticas e noutras doenças ósseas, como osteoporose e doença de Paget. O seu mecanismo de acção reduz a reabsorção óssea, a inibição do recrutamento e promoção da apoptose de osteoclastos. Apesar dos grandes benefícios para pacientes nestas condições, uma complicação associada ao seu uso é a osteonecrose dos maxilares. A osteonecrose maxilar associada ao tratamento com bisfosfonatos constitui uma complicação oral grave, que pode iniciar-se com osteomielite e progredir para a exposição crónica de osso necrosado. No caso particular dos doentes oncológicos submetidos a tratamento com bisfosfonatos, tem-se detectado vários casos de necrose óssea da mandíbula. Este assunto, apesar da sua baixa incidência e aparecimento relativamente recente, levou à introdução de linhas de conduta e recomendações internacionais para o uso de bisfosfonatos no tratamento de doentes oncológicos. Biphosphonates are a group of drugs used to treat metastases associated with breast or prostate cancer and other bone diseases, including osteoporosis and Paget disease. They inhibit bone resorption through an action on osteoclasts slowing their activity and inducing apoptosis. Despite the benefits of biphosphonates, osteonecrosis of the jaw (ONJ) has emerged as a serious complication in some patients treated with these drugs. Osteonecrosis of the jaw related to bisphosphonate therapy is a major oral complication that may begin with osteomyelitis and progress towards cronical necrotic bone exposure. There are several cases described of bone jaw necrosis, mainly in cancer patients treated with bisphosphonates. Although its low incidence, this emerging problem lead to the establishment of international guidelines for cancer patients that undergo bisphosphonate therapy.
