Abstract

Introdução: A deteção precoce da cárie dentária não cavitada é fundamental para o sucesso de intervenções minimamente invasivas. Métodos óticos, como a fluorescência e a transiluminação por luz próxima do infravermelho (NIR-TI), têm-se destacado como alternativas à radiografia digital, oferecendo maior sensibilidade e precisão diagnóstica em fases iniciais da doença. Materiais e métodos: Esta revisão sistemática seguiu as diretrizes PRISMA e foi registrada na base PROSPERO (CRD42024505959). Foram incluídos estudos clínicos comparativos in vivo realizados em dentes permanentes que avaliaram a eficácia diagnóstica da fluorescência e/ou da NIR-TI em relação à radiografia digital. Os principais outcomes considerados foram sensibilidade, especificidade, precisão e aplicabilidade clínica. Resultados: Foram incluídos quinze estudos para análise. A fluorescência mostrou elevada sensibilidade em superfícies oclusais; a NIR-TI apresentou bons níveis de precisão e sensibilidade para lesões interproximais. A radiografia digital, por sua vez, destacou-se pela especificidade. A combinação entre os métodos resultou numa maior eficácia diagnóstica. Conclusão: A utilização combinada de recursos de diagnóstico por imagem contribui para decisões clínicas mais seguras e conservadoras. Estudos futuros devem padronizar metodologias de investigação e avaliar os resultados destas tecnologias na prática clínica real e diária.

diária. Introduction: The early detection of non-cavitated dental caries is fundamental to the success of minimally invasive interventions. Optical methods, such as fluorescence and near-infrared light transillumination (NIR-TI), have gained prominence as alternatives to digital radiography, offering greater sensitivity and diagnostic precision in the early stages of the disease. Materials and Methods: This systematic review followed the PRISMA guidelines and was registered in the PROSPERO database (CRD42024505959). Comparative in vivo clinical studies conducted on permanent teeth that assessed the diagnostic efficacy of fluorescence and/or NIR-TI in relation to digital radiography were included. The main outcomes considered were sensitivity, specificity, precision, and clinical applicability. Results: Fifteen studies were included for analysis. Fluorescence showed high sensitivity on occlusal surfaces, while NIR-TI demonstrated good levels of precision and sensitivity for interproximal lesions. Digital radiography, in turn, was notable for its specificity. The combination of methods resulted in greater diagnostic efficacy. Conclusion: The combined use of imaging diagnostic resources contributes to safer and more conservative clinical decision-making. Future studies should standardize research methodologies and evaluate the outcomes of these technologies in real and daily clinical practice.
