El transportador de serotonina como diana terapéutica

Abstract

Debido a su amplia distribución en el encéfalo y a un elevado número de receptores, el sistema sertonérgico juega un papel crucial en la regulación de numerosas funciones fisológicas y participa de forma importante en procesos cognitivos y de control de los estados de ánimo. El transportador de serotonina es un elemento clave en el control de la actividad de este sistema de neurotransmisión, al regular la concentración efectiva de serotonina en la sinapsis del sistema nervioso. La máxima densidad del transportador de serotonina se encuentra en los núcleos del rafe del mesencéfalo, en especial el núcleo dorsal, donde se encuentran los somas de las neuronas serotonérgicas ascendentes. El bloqueo del transportador mediante inhibidores selectivos (citalopram, fluoxetina, fluvoxamina, paroxetina, sertralina)o no selectivos (p.ej., fármacos tricíclicos) produce un importante aumento de la concentración extracelular de serotonina en dichos núcleos, que es superior a los producidos en áreas de proyección, como la corteza, el hipocampo o el estriado. En éstas últimas no se producen aumentos de serotonina a dosis clínicamente relevantes. Esta selectividad regional parece estar involucrada en el retraso en la acción clínica de los antidepresivos y en su eficacia limitada. La aplicación de técnicas de genética molecular y de imaginería cerebral tomografía de emisión de positrones) al estudio del transportador de serotonina permitirá en un futuro no muy lejano un mejor conocimiento de las bases neurobiológicas de la acción de los antidepresivos en el cerebro humano.
