Fluctuating temperature modifies heat-mortality association around the globe

Abstract

Des études ont étudié les effets de la variabilité de la chaleur et de la température (TV) sur la mortalité. Cependant, peu d'entre eux ont évalué si la télévision modifiait l'association chaleur-mortalité. Les données sur la température et la mortalité quotidiennes pendant la saison chaude ont été recueillies dans 717 endroits de 36 pays. La TV a été calculée comme l'écart type de la moyenne des températures minimales et maximales des mêmes jours et des jours précédents. Nous avons utilisé des modèles de régression de quasi-Poisson spécifiques à l'emplacement avec un terme d'interaction entre le terme de base croisé pour la température moyenne et les quartiles de la télévision pour obtenir des associations de mortalité thermique sous chaque quartile de la télévision, puis des estimations regroupées aux niveaux national, régional et mondial. Les résultats montrent le risque accru de mortalité liée à la chaleur avec des augmentations dans la TV, représentant 0,70 % (intervalle de confiance à 95 % [IC] : -0,33 à 1,69), 1,34 % (IC à 95 % : -0,14 à 2,73), 1,99 % (IC à 95 % : 0,29-3,57) et 2,73 % (IC à 95 % : 0,76-4,50) du nombre total de décès pour le T1-Q4 (premier quartile-quatrième quartile) de la TV. Les effets de modification de la télévision variaient géographiquement. L'Europe centrale présentait les fractions attribuables (FA) les plus élevées, correspondant à 7,68 % (IC à 95 % : 5,25-9,89) du nombre total de décès pour le quatrième trimestre de la TV, tandis que les FA les plus faibles ont été observées en Amérique du Nord, avec des valeurs pour le quatrième trimestre de 1,74 % (IC à 95 % : -0,09 à 3,39). La télévision a eu un effet de modification significatif sur l'association chaleur-mortalité, provoquant une charge de mortalité liée à la chaleur plus élevée avec des augmentations de la télévision. La mise en œuvre simultanée de stratégies ciblées contre l'exposition à la chaleur et les températures fluctuantes serait bénéfique pour la santé publique.

Los estudios han investigado los efectos de la variabilidad del calor y la temperatura (TV) sobre la mortalidad. Sin embargo, pocos evaluaron si la TV modifica la asociación calor-mortalidad. Se recopilaron datos sobre la temperatura diaria y la mortalidad en la estación cálida de 717 ubicaciones en 36 países. La TV se calculó como la desviación estándar de la media de las temperaturas mínimas y máximas del mismo día y de los días anteriores. Utilizamos modelos de regresión cuasi-Poisson específicos de la ubicación con un término de interacción entre el término de base cruzada para la temperatura media y los cuartiles de TV para obtener asociaciones de mortalidad por calor en cada cuartil de TV, y luego combinamos estimaciones a nivel nacional, regional y global. Los resultados muestran el aumento del riesgo de mortalidad relacionada con el calor con incrementos en la TV, que representa el 0,70% (intervalo de confianza [IC] del 95%: -0,33 a 1,69), el 1,34% (IC del 95%: -0,14 a 2,73), el 1,99% (IC del 95%: 0,29-3,57) y el 2,73% (IC del 95%: 0,76-4,50) del total de muertes para el Q1-Q4 (primer cuartil-cuarto cuartil) de la TV. Studies have investigated the effects of heat and temperature variability (TV) on mortality. However, few assessed whether TV modifies the heat-mortality association. Data on daily temperature and mortality in the warm season were collected from 717 locations across 36 countries. TV was calculated as the standard deviation of the average of the same and previous days' minimum and maximum temperatures. We used location-specific quasi-Poisson regression models with an interaction term between the cross-basis term for mean temperature and quartiles of TV to obtain heat-mortality associations under each quartile of TV, and then pooled estimates at the country, regional, and global levels. Results show the increased risk in heat-related mortality with increments in TV, accounting for 0.70% (95% confidence interval [CI]: -0.33 to 1.69), 1.34% (95% CI: -0.14 to 2.73), 1.99% (95% CI: 0.29-3.57), and 2.73% (95% CI: 0.76-4.50) of total deaths for Q1-Q4 (first quartile-fourth quartile) of TV. The modification effects of TV varied geographically. Central Europe had the highest attributable fractions (AFs), corresponding to 7.68% (95% CI: 5.25-9.89) of total deaths for Q4 of TV, while the lowest AFs were observed in North America, with the values for Q4 of 1.74% (95% CI: -0.09 to 3.39). TV had a significant modification effect on the heat-mortality association, causing a higher heat-related mortality burden with increments of TV. Implementing targeted strategies against heat exposure and fluctuant temperatures simultaneously would benefit public health.
