Eficacia de Lactobacillus spp. en la inhibición de la adherencia de bacterias uropatógenas in vitro

Abstract

[Objetivos] En el inicio de las infecciones del tracto urinario (ITU), se produce la adherencia, a las células del urotelio, de bacterias uropatógenas de especies como Escherichia coli (UPEC) y Enterococcus faecalis. Entre las terapias alternativas al tratamiento de las ITU con antibióticos, y para evitar las resistencias bacterianas, se ha prescrito el consumo de arándano rojo como profiláctico. La administración de bacterias probióticas especialmente del género Lactobacillus se considera otra terapia prometedora en diversas patologías ginecológicas y en la prevención de las ITU. El objetivo de este trabajo es la evaluación in vitro de la posible inhibición por parte de Lactobacillus de la adherencia de uropatógenos a células del urotelio.

[Resultados] Las tres cepas de Lactobacillus presentaban altos valores de adherencia (>10,5%), especialmente L. acidophilus (12.9%), lo que sugería una alta afinidad a los receptores uroepiteliales. Las cepas uropatógenos mostraron porcentajes de adherencia entre 9,0% y 11,4%. La capacidad de excluir, competir y desplazar a los uropatógenos por los lactobacilli resultó elevada aunque muy variable entre las 3 cepas, siendo L. plantarum CLC 17 la cepa menos bloqueante. Se observaron mayores inhibiciones en la adhesión de E. faecalis y S. epidermidis (40%) que de las cepas UPEC, lo que puede ser debido a la capacidad adquirida de adherencia mediante las adhesinas de las fimbrias.

[Método] Se ha evaluado la adhesión a células epiteliales de vejiga T24 de diversas cepas de Lactobacillus (L. salivarius UCM572, L. plantarum CLC 17 y L. acidophilus), así como de la capacidad de las mismas para inhibir (ensayos de exclusión, competencia y desplazamiento) la adhesión de uropatógenos (E. coli ATCC®53503¿, E. coli 2980, E. faecalis 04-1, E. faecalis 08-1 y Staphylococcus epidermidis 08-3).

[Conclusiones] Los datos obtenidos apoyan que la ingesta de bacterias probióticas/lactobacilos podrían restablecer la microbiota e inhibir la adhesión de patógenos al urotelio y, por tanto, prevenir la incidencia/recurrencia de las ITU.
