Mieloesclerosis en una gata con linfoma/leucemia linfoblástica de células T

Abstract

En enero de 2018 se remitió para la necropsia una gata castrada, de raza Común Europeo, de 21 meses de edad, que fue remitida a una clínica veterinaria con palidez de mucosas, vómitos y mala condición corporal (2/5). El hematocrito (6,37%), hemoglobina (2,58 g/dl), número de glóbulos rojos (0,7x106 /mm3 ) y plaquetas (36x103 /mm3 ) eran muy inferiores a los normales, al contrario que los leucocitos (13,27x103 /mm3 ), linfocitos (6,8x103 /mm3 ), bilirrubina total (3,4 mg/dl) y transaminasas. Mediante ecografía se observó esplenomegalia. Dicho animal, negativo serológicamente a los virus de la inmunodeficiencia y leucemia felinas, falleció a las 8 horas de su ingreso en la clínica. En la necropsia se comprobó ictericia y hepatomegalia (“hígado azafranado”), esplenomegalia, con fractura esplénica, y hemoperitoneo. Los nódulos linfáticos gástricos, poplíteos e inguinales se encontraban aumentados de tamaño y la médula ósea tenía un color rojo y consistencia dura. En meninges y pulmón se observaron petequias y en el corazón hidropericardias con flóculos de fibrina. Microscópicamente en hígado, bazo, médula ósea y nódulos linfáticos destacaban infiltrados difusos de células voluminosas, esféricas, con núcleo redondo, cromatina finamente granular, nucléolo prominente y citoplasma relativamente abundante y basófilo. Eran frecuentes las mitosis, así como los fenómenos de apoptosis. Dichas células eran CD3 positivas y CD20 negativas. En la médula ósea el tejido hematopoyético aparecía sustituido por tejido fibroso (mieloesclerosis) y en el hígado (con colestasis y necrosis hepática centrolobulillar) y bazo se observó hematopoyesis extramedular. La mieloesclerosis es un proceso progresivo que suele estar asociado a esplenomegalia y hematopoyesis extramedular. La forma primaria es rara y la secundaria se ha relacionado con infecciones víricas o exposición a tóxicos que causan mielosupresión. En animales la leucemia linfoblástica de células T es un proceso rápido que cursa con anemia, trombocitopenia, fallo de la médula ósea y hepatoesplenomegalia.
