Influencias de determinantes socio-económicos en la elección de la terapia sustitutiva renal

Abstract

En este estudio descriptivo realizado en el departamento de salud de Castellón, se investigó la relación entre factores socioeconómicos y demográficos y la elección de las modalidades de tratamiento en pacientes con enfermedad renal crónica avanzada. Se incluyeron 200 pacientes, pudiendo finalmente analizar datos de 59. Estos datos se recogieron mediante una encuesta que abordaba variables como la modalidad de tratamiento, nivel socioeconómico, ingresos mensuales, nivel educativo y apoyo social. Los objetivos fueron describir el perfil socioeconómico y demográfico de los pacientes, evaluar la relación entre estas variables y la elección de modalidades de tratamiento, y recopilar los motivos de elección de los pacientes. Los resultados reflejaron que la mayoría de participantes recibía tratamiento hospitalario (64.4%), seguido de tratamiento domiciliario (18.6%) y conservador (16.9%). La distribución de género fue equitativa (50.8% hombres, 49.2% mujeres), y la edad promedio fue 71.76 años. Se encontraron relaciones entre la elección del tratamiento y variables como acceso a internet en casa, espacio para almacenar el material de diálisis, número de personas en el hogar y edad del paciente. Los motivos principales que refirieron los pacientes para la elección fueron recomendación médica, acompañamiento del personal sanitario y comodidad. La autonomía, flexibilidad y estabilidad de la enfermedad renal también influyeron. El estudio tiene limitaciones debido al tamaño de la muestra y la recopilación de datos, pero resalta la importancia de considerar factores socioeconómicos y demográficos al decidir el tratamiento. Se necesita garantizar un acceso equitativo y proporcionar información adecuada a los pacientes.

In this descriptive study conducted at the Department of Health in Castellon, the relationship between socioeconomic and demographic factors and the choice of treatment modalities in patients with advanced chronic kidney disease was investigated. A total of 200 patients were included, and data from 59 patients were finally analysed. These data were collected through a survey that addressed variables such as treatment modality, socioeconomic level, monthly income, educational level, and social support. The objectives were to describe the socioeconomic and demographic profile of the patients, evaluate the relationship between these variables and the choice of treatment modalities, and collect the reasons for the patients' choices. The results showed that the majority of participants received hospital-based treatment (64.4%), followed by home-based treatment (18.6%) and conservative treatment (16.9%). The gender distribution was balanced (50.8% men, 49.2% women), and the average age was 71.76 years. Relationships were found between treatment choice and variables such as access to internet at home, space for storing dialysis materials, number of people in the household, and patient's age. The main reasons cited by patients for their treatment choice were medical recommendation, support from healthcare staff, and convenience. Autonomy, flexibility, and stability of kidney disease also influenced the decision. The study has limitations due to the sample size and data collection, but it highlights the importance of considering socioeconomic and demographic factors when deciding on treatment. Ensuring equitable access and providing adequate information to patients is necessary.
