Introducción: El fracaso renal agudo (FRA) es una complicación frecuente en los pacientes de las Unidades de Cuidados Intensivos (UCI) y muchos requieren técnicas de reemplazo renal (TRR). La lesión renal aguda puede inducir daño renal crónico irreversible en pacientes sanos y acelerar la progresión de la enfermedad renal entre pacientes con enfermedad renal crónica preexistente, que se asocia con un aumento de la mortalidad hospitalaria. Objetivos: Valorar la incidencia del FRA y de necesidad de tratamiento con TRR en los pacientes ingresados en la UCI, analizar los factores independientes relacionados con la necesidad de TRR en UCI y la supervivencia y estancia en UCI y hospitalaria. Métodos: Estudio de cohorte retrospectivo realizado en la UCI del Hospital General Universitario de Castellón (HGUCS) entre enero y diciembre de 2018. Se analizaron variables demográficas, índices de gravedad, variables clínicas y de laboratorio al ingreso, al diagnóstico del FRA y al alta hospitalaria. Resultados: En 89 pacientes (5,93%) se diagnosticó FRA y, 38 de éstos (42,7 %) fueron tratados con TRR. Al alta del hospital 8 pacientes continuaban necesitando diálisis, con diferencias significativas en las cifras de creatinina entre los tratados con hemodiálisis intermitente (HDI) en UCI y aquellos que no, 4,75 3,63 vs 0,81 (0,70- 1,39) p 0,044. Conclusiones: El FRA es una entidad infradiagnosticada en la UCI del HGUCS, la presencia de FRA con necesidad de TRR se asocia a lesión renal crónica, necesidad de diálisis al alta hospitalaria, mayor mortalidad y estancia en UCI y hospitalaria.
Introduction: Acute renal failure (ARF) is a frequent complication in patients in Intensive Care Units (ICU) and many require renal replacement techniques (RRT). Acute kidney injury can induce irreversible chronic kidney damage in healthy patients and accelerate the progression of kidney disease. This is associated with an increased in hospital mortality. Objectives: To assess the incidence of ARF and the need for RRT treatment in patients admitted in the ICU, to analyse the independent factors related to the need for RRT treatment in the ICU and the survival and stay in the ICU and hospital. Methods: A retrospective cohort study carried out in the ICU of the General Universitary Hospital of Castellon (between January 2018 to December 2018. Demographic variables, severity scores and clinical and laboratory variables were assessed at admission, diagnosis of ARF and hospital discharge. Results: In 89 patients (5,93%) ARF was diagnosed and 38 of these (42,7%) were treated with RRT. At hospital discharge, 8 patients needed dialysis, with significant differences in creatinine levels between those treated with intermittent hemodialysis (HDI) in the ICU and those who not, showed statistically significant differences 4,75 3,63 vs 0,81 (0,70-1,39) p 0,044. Conclusions: ARF is an underdiagnosed entity in the ICU of HGUCS, the presence of ARF with the need of the RRT is associated with chronic kidney injury, the need for dialysis at hospital discharge and higher mortality and ICU and hospital stay.
