Satisfacción del paciente intervenido de artroplastia de rodilla - II

Abstract

Treball Final de Grau en Medicina. Codi: MD1158. Curs acadèmic: 2018/2019. Introduction. The purpose of this study was to analyse the satisfaction in patients whose total knee artroplasties (TKA) surgeries were carried out in Hospital Provincial, in Castellón, between January 2015 and December 2017. The hypothesis of the study was to prove that sex, age, lack of obesity, diabetes, mental health problems, as well as perfect results in clinical examination, radiological findings, pain assessment and patient reported outcomes measures (PROMS) are related to good satisfaction. Material and methods. This observational and cross-sectional study included 30 patients who undergone a TKA surgery due to primary knee osteoarthritis. Data collection was obtained from patient interview and clinical assessment. In addition, their medical history was examined too. In order to make that, we used some analytic tools, such as VAS (visual analogue scale) to pain assessment, MRC scale to check the muscular strength or patient reported outcomes measures (PROMS) like KOOS. SPSS 22.0 software was employed to data analysis. Results. The statistical analysis didn’t prove connection between patients’ satisfaction and the rest of the variables. However, we could apreciate that if our sample was higher, the significance would be suitable. Conclusion. The patients’ satisfaction after TKA is very difficult to assess because it depends on many objective and subjective factors. In this study, we couldn’t prove that secondary variables were related to the main variable. Introducción. La finalidad de este estudio fue analizar la satisfacción de los pacientes intervenidos de artroplastia de rodilla en el Hospital Provincial de Castellón desde Enero de 2015 a Diciembre de 2017. La hipótesis del estudio fue demostrar que el sexo, la edad, la ausencia de obesidad, diabetes y problemas de salud mental previamente a la cirugía, así como resultados idóneos tanto clínicos, radiológicos como de cuestionarios de calidad de vida relacionada con la salud, se relacionan con una buena satisfacción del paciente. Material y método. Se trata de un estudio de tipo observacional y transversal que incluyó 30 pacientes intervenidos de artroplastia de rodilla por artrosis primaria. Los datos fueron recogidos mediante una entrevista y exploración física al paciente, así como, con la revisión de sus historias clínicas. Para ello nos ayudamos de algunas herramientas como las escalas EVA y MRC para evaluar el dolor y la fuerza muscular respectivamente, o el cuestionario KOOS para evaluar la calidad de vida. El análisis estadístico se realizó con la versión 22.0 del SPSS. Resultados. El análisis estadístico no mostró relación significativa entre la satisfacción y el resto de variables estudiadas, pero en algunas variables se apreció una tendencia a la significación con un tamaño mayor de muestra. Conclusión. La satisfacción del paciente tras una artroplastia total de rodilla es compleja de evaluar porque depende de muchos parámetros, tanto objetivos como subjetivos. En este estudio, no se ha podido demostrar que las variables secundarias estén relacionadas con la variable principal.
