La sinusitis aguda es una enfermedad infecciosa que generalmente se alivia espontáneamente. La aparición de complicaciones es poco fre - cuente; sin embargo, pueden ser graves por la proximidad de los senos paranasales con el sistema nervioso central. Puede ocurrir una extensión intracraneal del proceso infeccioso, desarrollar síntomas neurológicos y comprometer la vida del paciente. Es importante sospechar este tipo de complicación ante la aparición de un cuadro clínico neurológico durante la evolución de una sinusitis, así como realizar diagnóstico y tratamiento adecuados. Las pruebas de imagen son de gran utilidad en estos casos pues permiten el estudio de las estructuras intracraneales. Se presenta un caso de sinusitis complicada, se repasan las principales complicaciones intracraneales y sus hallazgos radiológicos en tomogra - fía computada y resonancia magnética con la finalidad de establecer un diagnóstico objetivo, preciso y oportuno. De ese modo es posible instaurar un tratamiento temprano y disminuir notablemente la morbi - lidad y la mortalidad asociadas.
Acute sinusitis is an infectious disease which usually resolves spon- taneously. The appearance of complications is uncommon; however, they can be serious due to the proximity of the paranasal sinuses to the central nervous system. It may lead to intracranial spread of the infectious process or development of neurological symptoms, and can compromise the patient’s life. It is important to suspect this kind of complications if neurological clinical manifestations appear during the evolution of a sinusitis, and perform appropriate diagnosis and treatment. Image tests are highly useful in such cases as they allow us to study the intracranial structures. We present a case of complicated sinusitis, and review the principal intracranial complications and their radiological findings in computed tomography and magnetic resonance to establish an objective, precise, and opportune diagnosis. Thus, it is possible to institute early treatment and substantially reduce associated morbidity and mortality.
