Efecto de pastas y barnices de aplicación dental sobre la remineralización del esmalte

Abstract

El desarrollo de la Odontología Mínimamente Invasiva ha impulsado los tratamientos remineralizantes de las desmineralizaciones del esmalte. El objetivo de este estudio, fue comparar la capacidad remineralizante de ocho productos comerciales: MI Varnish™ (CPP-ACP), Clinpro™ White Varnish (β-TCP, NaF), Bifluorid 12(5% NaF), Tooth mouse™ (CPP-ACP), MI Paste Plus™ (CPP-ACPF), Remin Pro® (HA, NaF, xilitol), Clinpro™ Tooth Crème (β-TCP, NaF) y Colgate® Sensitive Pro-Alivio (NaF, arginina). Para la consecución del trabajo se emplearon 280 dientes bovinos a los que se evaluó el contenido mineral basal, tras la desmineralización y tras la aplicación de los productos remineralizantes. Las muestras se dividieron aleatoriamente en ocho grupos, cinco en los que se aplicaron pastas dentales y tres tratados con barnices: grupo 1 Tooth mouse™ (CPP-ACP), grupo 2 MI Paste Plus™ (CPP-ACPF), grupo 3 Remin Pro® (HA, NaF, xilitol), grupo 4 Clinpro™ Tooth Crème (β-TCP, NaF), grupo 5, Colgate® Sensitive Pro-Alivio (NaF, arginina), grupo 6 MI Varnish™ (CPP-ACP), grupo 7 Clinpro™ White Varnish (β-TCP) y grupo 8 Bifluorid 12 (5% NaF). El análisis de la remineralización se evaluó tras 7, 14 y 28 días mediante el empleo de rayos X de Energía Dispersiva (EDX) y Microscopio Electrónico de Barrido (MEB). Los resultados del análisis de EDX mostraron un descenso significativo en el Ca y P tras la aplicación del agente desmineralizante (p<0.05). Tanto en el grupo de pastas como en el de barnices, apareció un incremento significativo de las concentraciones de Ca y P a partir de los 14 y 28 días (p<0.05), con pequeñas variaciones según el grupo de estudio. La ratio Ca/P mostró un descenso tras la desmineralización, seguido de un aumento progresivo hacia el día 28. En las imágenes obtenidas mediante el MEB se observaron microporosidades tras la desmineralización e imágenes características de las diferentes pastas y barnices sobre la superficie del esmalte en las que se ha observado un recubrimiento progresivo de los micropocillos de la desmineralización. En conclusión, los ocho productos estudiados “in vitro” muestran una alta capacidad remineralizante, permitiendo recuperar valores minerales similares a los basales tras su aplicación durante un periodo de aplicación de cuatro semanas. The Minimally Invasive Dentistry development has fostered remineralizing treatments of demineralised enamel. The objective of this study was to compare the remineralizing capacity of eight commercial products: MI Varnish™ (CPP-ACP), Clinpro™ White Varnish (β-TCP, NaF), Bifluorid 12(5% NaF), Tooth mouse™ (CPP-ACP), MI Paste Plus™ (CPP-ACPF), Remin Pro® (HA, NaF, xilitol), Clinpro™ Tooth Crème (β-TCP, NaF) y Colgate® Sensitive Pro-Alivio (NaF, arginina). For the attainment of the study, 280 bovine teeth were used and mineral content was evaluated before and after being demineralized and after the application of the remineralizing products. Samples were randomly divided into eight groups, five treated with toothpaste and three with varnishes: group 1 Tooth mouse™ (CPP-ACP), group 2 MI Paste Plus™ (CPP-ACPF), group 3 Remin Pro® (HA, NaF, xilitol), group 4 Clinpro™ Tooth Crème (β-TCP, NaF), group 5, Colgate® Sensitive Pro-Alivio (NaF, arginina), group 6 MI Varnish™ (CPP-ACP), group 7 Clinpro™ White Varnish (β-TCP) y group 8 Bifluorid 12 (5% NaF). Remineralization analysis was performed 7, 14 and 28 days after their application by the use of Energy Dispersive X-ray (EDX) and Scanning Electron Microscope (SEM). The results of EDX analysis showed a significant decrease in Ca and P levels after application of the demineralizing agent (p<0.05). A significan increase in Ca and P concentrations after 14 and 28 days (p<0.05) was observed in paste and varnish groups, with small variations according to the study group. The Ca/P ratio showed a decrease after demineralization, followed by a progressive increase towards day 28, significant in some cases. In the SEM images, microporosities were observed after demineralization and characteristic images of the different pastes and varnishes on the enamel surface showed a progressive coating of the demineralization microwells. In conclusion, the eight products studied "in vitro", show a high remineralizing capacity, allowing to recover mineral values similar to the basal ones after their application during a period of application of four weeks.
