Abstract

Introducción: La Diabetes mellitus aumenta el riesgo de IU en las mujeres diabéticas afectando su calidad de vida. Objetivos: Evaluar la prevalencia de IU, y su relación con variables sociodemográficas y clínicas, en una muestra de mujeres diabéticas de 18 a 64 años, así como la afectación de la calidad de vida y su comparación las mujeres de la población general. Metodología: Estudio descriptivo transversal en una muestra de 291 mujeres atendidas en un centro de salud de la ciudad de Alicante. Mediante entrevista clínica se recogieron datos de síntomas y severidad de la IU (International Consultation on Incontinence Questionnaire (ICIQ-UI SF) e Índice de Severidad de Sandvik), calidad de vida (SF-36) y otras variables clínicas y sociodemográficas. Resultados: La prevalencia de IU fue del 54,3%, alcanzando el 64,9% en el grupo de 45 a 54 años. Es más elevada a menor nivel de estudios, mayor índice de masa corporal, diabetes tipo 2 y sin tratamiento dietético. La incontinencia urinaria mixta (IUM) es la más prevalente (53,2%), especialmente en obesas. La calidad de vida es inferior en las mujeres incontinentes, principalmente en la IUM. Los síntomas de incontinencia tienen una antigüedad media de 8,0 + 9,3 años, y han buscado ayuda el 24,8% de las pacientes, ninguna de ellas al fisioterapeuta. Al 32,8% de las pacientes, la incontinencia les ocasiona más problemas en su vida diaria que la diabetes, y al 30% le dificulta realizar ejercicio físico. Conclusiones: La prevalencia de IU en las mujeres diabéticas es mayor que la de la población general, y su calidad de vida es menor que en las continentes y en las de la población general, con impacto más negativo en la IUM. La solicitud de ayuda profesional para la IU es deficiente. Purpose: To assess the prevalence of UI, and its relationship to socio-demographic and clinical variables in a sample of diabetic women of 18-64 years, as well as the effects on the quality of life and their comparison with women in the general population. Methods: Cross-sectional study in a sample of 291 women attending a health centre in the city of Alicante. Though clinical interviews, data were collected about symptoms and severity of UI (International Consultation on Incontinence Questionnaire (ICIQ-UI SF) and Sandvik IU Severity Index), quality of life (SF-36) and other clinical and socio-demographic variables. Results: The prevalence of UI was 54.3%, reaching 64.9% in the group of 45-54 years. It is higher in patients with lower levels of education, higher body mass index, type 2 diabetes and no dietary treatment. Mixed urinary incontinence (MUI) is the most prevalent (53.2%) especially in obese women. The quality of life is lower in incontinent women, especially in those with MUI. Symptoms of incontinence had an average duration of 8.0 + 9.3 years, and 24.8% of patients sought help, none of them from a physiotherapist. In 32.8% of patients, incontinence causes more problems in their daily lives than diabetes, and for 30% it makes exercise difficult. Conclusion: The prevalence of UI in diabetic incontinent women is higher than that of the general population, and the quality of life is lower than in diabetic continents and the general population, with more negative impact in those affected by MUI.
