Abstract

La mayoría de los síndromes vertiginosos constituyen procesos benignos que se resuelven con o sin tratamiento en poco tiempo, pero existe la posibilidad de que se trate de un síntoma de una enfermedad neurológica importante. En particular, los vértigos periféricos aparecen por lesión de los receptores vestibulares (laberinto) o del nervio estatoacústico. Entre las pruebas de mayor utilidad clínica para documentar la existencia de una lesión del nervio estaoacústico se destacan los Potenciales Evocados Auditivos de Tallo Cerebral (PEATC), los cuales pueden a su vez ofrecer información indirecta sobre el componente vestibular. Se estudiaron 37 pacientes hospitalizados o provenientes de la consulta externa del Centro Internacional de Restauración Neurológica remitidos a nuestro servicio con el diagnóstico de síndrome vertiginoso de causa periférica. A los mismos se realizó el PEATC, siguiendo la rutina establecida en el manual de procedimientos del laboratorio, previa evaluación del umbral subjetivo de audición. Encontramos que el estudio fue positivo en el 78,38 % de los casos estudiados y que el mayor porcentaje de anomalías estuvo relacionado con la amplitud de la onda I (lo que sugiere un daño axonal del nervio estatoacústico), en tanto que los parámetros latencia absoluta de las ondas e intervalos interpicos prácticamente no mostraron variaciones. Nuestros resultados confirmaron la existencia de anomalías funcionales en el nervio estatoacústico de los pacientes estudiados con síndrome vertiginoso periférico.

ABSTRACT Most vertigo syndromes are benign processes that are resolved with or without treatment in a short period of time, but there is the possibility that the symptoms may represent an important neurologic disease. Specifically, periphery vertigos appear due to a lesion in the vestibular receptors (labyrinths) or in the statoacoustic nerve. Auditory Brainstem Response is among the most useful clinical tests to document the existence of a lesion of the statoacoustic nerve. In turn, it may offer information about the vestibular component. Thirty-seven inpatients or patients coming from the office of International Center for Neurological Restoration were sent to our service diagnosed with vertiginous syndrome of a peripheral cause. These patients were given ABR, following the established routine from the laboratory-procedure manual, previous the subjective evaluation of the hearing threshold. It was found that such study was positive in 78.38% of studied cases, and a greater percentage of anomalies were related to wave-amplitude (which suggests an axonal damage of the statoacoustic nerve); whereas parameters of absolute wave-latency and interpeak intervals practically did not show any variations. Our results confirmed the existence of functional anomalies in the statoacoustic nerve of studied patients with peripheral vertigo syndrome.
