Os autores discutem as indicações atuais da endarterectomia de carótida com base nas recomendações da American Heart Association (AHA). São revisados os principais estudos clínicos prospectivos multicêntricos internacionais em pacientes sintomáticos e assintomáticos e que sustentam as recomendações atuais. Mesmo com os recursos diagnósticos mais recentes, a indicação cirúrgica tem por base o grau da estenose independente das características da placa. A endarterectomia da carótida está indicada, tanto nos pacientes sintomáticos como nos assintomáticos, somente nas estenoses acima de 70%.
The current indications for carotid endarterectomy based on the guidelines ofthe Stroke Council, American Heart Association are discussed. The symptomatic and asymptomatic randomized prospective clinical trials support the AHA guidelines. The surgical indicatiol1s are based on the degree of stenosis with 01' without ulceration. Carotid endarterectomy is indicated at our institutionfor patient with symptomatic 01' asymptomatic stenotic lesions exceeding 70%.
