Trombose da veia porta em crianças e adolescentes : série de 14 casos

Abstract

Objetivo: Os autores analisam uma casuística de 14 crianças e adolescentes com trombose da veia porta atendidas no ambulatório de Gastroenterologia Pediátrica do Hospital de Clínicas de Porto Alegre. Métodos: O diagnóstico da trombose foi realizado por ultra-sonografia abdominal com Doppler e/ou fase venosa do angiograma celíaco seletivo. Os pacientes apresentavam testes hepáticos normais e biópsia do fígado sem alterações compatíveis com doença hepática. Resultados: Os pacientes eram caucasóides e metade pertencia ao gênero feminino. A média de idade por ocasião do início doestudo era de 8 anos e 8 meses, e ao diagnóstico da trombose, de 3 anos e 8 meses. As manifestações clínicas iniciais foram hemorragia digestiva alta em 9/14 (64,3%) e achado de esplenomegalia ao exame físico em 5/14 (35,7%). Anomalias congênitas extra-hepáticas foram identificadas em 3/14 (21,4%) e fatores locais (cateterismo umbilical, onfalite e septicemia) em 5/14 (35,7%) dos pacientes. Nenhum dos pacientes apresentava consangüinidade ou história familiar de trombose venosa. Conclusão: A trombose da veia porta deve ser suspeitada em toda criança e adolescente com hemorragia digestiva alta e esplenomegalia sem estigmas de hepatopatia crônica. A presença de intercorrências no período neonatal e anomalias extra-hepáticas devem ser investigadas. A associação de trombose em outros locais, história familiar de trombose e consangüinidade não foi observada.

ObjecJjve: The authors analyze 14 children and adolescents with portal vein thrombosis admitted in the Division of Pediatric Gastroenterology of Hospital de Clínicas de Porto Alegre. Methods: Portal vein thrombosis was diagnosed through abdominal Doppler ultrasonography andJor venous phase of selective coeliac angiograms. Patients presented normalliver tests and hepatic biopsy (7/14), with no signs of liver disease. Results: 14 Caucasian patients were assessed. Halfofthe patients were females. The mean and standard deviationfor age were 8 years and 8 months while the mean standard deviation for diagnosis were 3 years and 8 months. 1nitial clinicai manifestations upon diagnosis were digestive hemorrhage in 9/14 (64,3%) and splenomegaly on physical examination in 5/14 individuais (35,7%). Patients presented extrahepatic anomalies in 3/14 (21,4%) and acquired riskfactors in 5/14 (35,7%) ofthe cases. None ofthe patients had afamily history of consanguinity or venous thrombosis. Conclusions: Portal vein thrombosis should be suspected in any child with gastrointestinal hemorrhage and splenomegaly without stigmas ofchronic liver disease. The presence ofevents during the neonatal period and extrahepatic anol1lalies should be investigated in these patients. The associaiion with throl1lbosis in other sites andfal1lily history of consanguinity or venous throl1lbosis was not observed.
