Abstract

A insuficiência ventricular esquerda com fração de ejeção normal – insuficiência cardíaca diastólica – apresenta uma prevalência ao redor de 30 a 40%. O ecocardiograma por Doppler emergiu como o método não-invasivo padrão para avaliar a função diastólica ventricular esquerda, especialmente através do Doppler pulsado mitral (DPM) e do Doppler pulsado tecidual (DPT). Ambos, embora mais notadamente o DPM, são influenciados pela pré-carga. Os pacientes com insuficiência renal crônica em hemodiálise (HD) freqüentemente apresentam disfunção diastólica. A possibilidade de realização de DPM e DPT, antes e após HD, permite a avaliação da função diastólica em diferentes situações de carga volêmica, estimando melhor os efeitos sobre a mesma de alterações na pré-carga e, ao mesmo tempo, considerando a capacidade do DPT, dito menos dependente da carga, na diferenciação de graus diversos de disfunção diastólica, quando comparado ao DPM. Vários trabalhos neste sentido têm sido encontrados na literatura. Na mesma linha, nossa experiência tem demonstrado que o DPT, para avaliar a função diastólica em pacientes renais crônicos antes e após HD, quando associado a manobra para aumento da pré-carga, dado não considerado em relatos anteriores, mostra-se um método útil e acessível para identificar e estratificar mais acuradamente graus diversos de disfunção diastólica, desde que as medidas sejam obtidas naqueles pacientes com volemia normal ou quase normal.

Left ventricular failure with normal ejection fraction, diastolic heart failure, has a prevalence of 30-40%. The pulsed-wave Doppler echocardiography has become the standard noninvasive method for evaluating the left ventricular diastolic function, particularly through transmitral pulsedwave Doppler (MPD) and tissue Doppler imaging (TDI). Both show characteristics of preload dependence, mainly the MPD. Chronic renal failure patients undergoing hemodialysis often have diastolic dysfunction. Obtaining MPD and TDI measurements before and after hemodialysis allows the evaluation of diastolic function under different conditions of volemic load. The effects of preload changes on diastolic function can be better estimated. At the same time, the ability of TDI, which is considered a less load-dependent method, to stratify different degrees of diastolic function compared with MPD can be studied more accurately. Several studies about this topiccan be found in the literature. Our experience has shown that the TDI associated with a maneuver to increase preload might be a useful and feasible method to identify and stratify diastolic dysfunction in renal failure patients before and after hemodialysis. This specific method of evaluation, which was not considered in previous studies, can be achieved when measurements are obtained from patients with normal or near normal volemic load.
