Abstract

A miocardiopatia dilatada é uma doença de caráter complexo e multifatorial, em que infecções virais, mecanismos auto-imunes e fatores genéticos produzem lesão miocárdica definitiva. É diagnosticada habitualmente na forma avançada, na qual determina severo comprometimento da função sistólica do ventrículo esquerdo e manifestações clínicas de insuficiência cardíaca. Pode ser identificada em forma inicial, caracterizada pela ausência de sinais congestivos e leve disfunção sistólica. Distúrbios do enchimento diastólico do ventrículo esquerdo são evidenciados em ambas as formas. A biópsia endomiocárdica identifica alterações histopatológicas, as quais não se correlacionam com o grau de comprometimento da função ventricular esquerda.

Dilated cardiomyopathy is a complex and multifactorial disease, in which viral infections, autoimmune mechanisms and genetic factors cause permanent myocardial damage. It is usually diagnosed in advanced forms, in which it determines severe impairment of the left ventricular systolic function and clinical signs of heart failure. It may be identified at an early stage, characterized by the absence of congestive signs and mild systolic dysfunction. Left ventricular diastolic filling abnormalities are found in both clinical forms. The endomyocardial biopsy shows histopathologic abnormalities that do not correlate with the degree of left ventricular function impairment.
