Tomografía de emisión de positrones con F-18-FDG: una nueva técnica en la evaluación de pacientes con neoplasias de cabeza y cuello

Abstract

Introducción. La tomografía de emisión de positrones con flúor-desoxiglucosa (PET-FDG) es una técnica eficaz en la detección y estadiaje de tumores debido a su elevado metabolismo glicolítico. El objetivo de este trabajo fue evaluar la PET-FDG en la detección de metástasis así como de enfermedad residual o recidiva en pacientes con tumores de cabeza y cuello. Material y Métodos. Se realizó PET-FDG en 11 pacientes remitidos para estadiaje (grupo I) y en 37 pacientes tratados con intención curativa y remitidos por sospecha de enfermedad residual o recidiva (grupo II) . Los estudios PET-FDG fueron comparados con los resultados obtenidos con la tomografía axial computarizada (TAC) en 35 pacientes, la resonancia magnética nuclear (RMN) en 4 pacientes y con ambas técnicas en 6 pacientes. Resultados. La PET-FDG fue positiva en todos los pacientes del grupo I, mientras que la TAC fue negativa en tres casos. En el grupo II la PET-FDG identificó con precisión recurrencia en 22 de 25 pacientes, mientras que la TAC y RMN fueron negativas o no concluyentes en 10 casos. Sin embargo, hubo un falso diagnóstico positivo PET con TAC no concluyente en un paciente con infección local. Conclusión. La PET-FDG fue altamente eficaz en la detección de metástasis locorregionales y a distancia, siendo además muy útil en la diferenciación entre enfermedad residual o recurrencia frente a secuelas del tratamiento cuando la TAC o RMN fueron no concluyentes debido a la distorsión anatómica.
