Abstract

La restauración de la acomodación perdida tras la extracción de la catarata es uno de los retos de la Oftalmología actual. La cirugía moderna de cristalino debería tener como objetivo encontrar un método seguro y eficaz para recuperar la habilidad de enfocar de forma dinámica, con un rango de visión continua desde la visión lejana a la cercana, pasando por la intermedia. La mayoría de los procedimientos actuales mejoran la visión próxima gracias al efecto de distintos factores pseudoacomodativos, sin restaurar el verdadero mecanismo de acomodación. Las lentes multifocales son un gran avance, mejorando en gran medida la visión en distancias cortas, pero sacrificando en parte la calidad visual debido al enfoque simultáneo de varias imágenes en la retina. Por este motivo surgen las lentes intraoculares acomodativas que pretenden con sus variados mecanismos de acción, como el desplazamiento axial o la incurvación de su óptica, conseguir un cambio óptico dinámico en el poder refractivo del ojo pseudofáquico, imitando el comportamiento del ojo fáquico joven para enfocar en todas las distancias con la calidad de una lente monofocal.
