Abstract

El objetivo de esta revisión es examinar los problemas de delimitación diagnóstica del síndrome bulímico, de categorías clínicas limítrofes: anorexia nerviosa e ingesta compulsiva. Existen diferencias entre bulimaréxicas y bulímicas, y entre bulimaréxicas y anoréxicas restrictivas, que impiden homologar a las bulimaréxicas, a las anoréxicas restrictivas o a las bulímicas de peso normal. La alternativa más prudente hasta que no se obtenga evidencia empírica que aproxime a las bulimaréxicas en una dirección o en otra, es asignar un diagnóstico mixto de anorexia y bulimia nerviosa a pacientes con sintomatología bulímica y anoréxica concurrentes. Los obesos que presentan episodios de hiperfagia, muestran diferencias respecto a las bulímicas en términos de precipitantes de las comilonas, consecuentes emocionales de éstas y topografía, peso y psicopatología. Estas diferencias aconsejan la inclusión de una categoría diagnóstica aparte para este subgrupo de obesos.

The aim of this review is to examine diagnostic delimitation problems of bulimic syndrome, from borderline clinic categories: anorexia nervosa and binge eating disorder. There are some differences between bulimic anorexic and bulimic women and, between bulimic anorexic and restricting anorexic women. This precludes to match bulimic anorexic to restricting anorexic women or bulimic of normal weight. There isn't empiric evidence that aproximates bulimic anorexic to bulimic or to restricting anorexic. The most cautious option is to give a mixed diagnostic of anorexia and bulimia nervosas at patients with concurrent bulimic and anorexic symptomathology. The obese subjects presenting binge eating episodes, show some differences to, bulimic women such as precipiting of binges, emotional consequences, topographic features of binges, weight and psychopathology. This differences suggest the addition of a separated diagnostic category for this obese subgroup.
