Terapia de desbridamiento larval. Revisión sistemática

Abstract

Objetivo principal: identificar y describir la evidencia científica de la terapia larval para el tratamiento de las heridas crónicas. Objetivo secundario: evaluar los efectos beneficiosos de la terapia larval, así como la capacidad de desbridamiento, desinfección y promoción de la cicatrización. Metodología: Se ha realizado una revisión bibliográfica, para poder localizar y recuperar la información relevante sobre la terapia larval. Para ello se han utilizado las siguientes bases de datos: Medline (a través de PubMed), Lilacs, Cinhal y Dialnet. La selección de artículos se ha llevado a cabo siguiendo los criterios de inclusión. Resultados: Finalmente se seleccionaron 26 artículos, entre ellos revisiones bibliográficas, casos clínicos, estudios de cohortes, meta-análisis, ensayos clínicos aleatorizados y estudios prospectivos. Los resultados a los que se ha llegado son los siguientes: la terapia larval se relaciona con tres acciones: actividad desbridante, antimicrobiana y promotora del tejido de granulación. A veces suele causar rechazo por parte de los pacientes y en ocasiones produce dolor. Conclusiones: la terapia larval es útil como método de desbridamiento en heridas, además de poseer otros beneficios como las propiedades antibacterianas de las larvas, la regeneración y la promoción del tejido de granulación. Es un terapia simple y costo-efectiva.
