Legemiddelbehandling av eldre pasienter med demenssykdom i sykehjem. En pilotstudie ved et sykehjem i Tromsø

Abstract

Gjennomgang av medikamentlistene til 29 pasienter ved et sykehjem i Tromsø viser et gjennomsnittlig antall faste forskrivninger per beboer på 6,4. Totalantallet forskrivninger (inkludert behovsmedisin) er 15,3. 41 % av pasientene bruker en eller flere typer nevroleptika fast, og 76 % har en eller flere typer nevroleptika som behovsmedisin. Datagrunnlaget er for lite til å si noe sikkert om den generelle medikamentbruken ved norske sykehjem; men sammenligning med en tidligere større studie antyder at fast bruk av nevroleptika, og spesielt antipsykotika, er redusert. Forskrivning av anxiolytika og hypnotika som behovsmedisin har derimot økt. I følge sykepleierne på sykehjemmet, er de største utfordringene ved legemiddelbehandling av eldre med demenssykdom: Vurdering av behandlingseffekt og bivirkninger, og den praktiske gjennomføringen av legemiddeladministrasjon. Tidligere ble legemiddelgjennomganger sjelden gjort, men i den senere tid oppleves et økt fokus på riktig legemiddelbehandling, spesielt med oppstart av prosjektet Kvalitetssikring av tverrfaglig legemiddelgjennomgang i Tromsø kommune.
