Nowadays, smoking is one of the most important causes of disease in our society; every year, more than 1 million people die because of secondhand smoke exposure. Protecting young people (because they represent a group in risk) should be our goal. The Spanish Network of Health-Promoting Universities (REUPS) partners with other organizations to support the creation of smoke-free campus. The main objective was to find out the prevalence and type of smoke-free actions created at “healthy universities”, and to get to know how they carry them out. We used a prevalence study which investigated the 60 universities that belonged to REUPS during the 2024–25 academic year with an online, multiple-choice, anonymous, and voluntary survey between February 14 to 28, 2025. We analyzed the results from 57 universities (36 of them via survey and the other 21 from official sources). 78.9% were public and Andalucia was the community with the biggest participation. 75% of universities developed actions related to smoke-free outdoor places. The measures that were the most common were educational activities, awareness campaigns about environmental tobacco smoke, and smoke-free signaled places. Less than half of the universities reported a 100% smoke-free campus. In conclusion, in order to achieve 100% smoke-free campus smoke-free policies must be stronger through collaborative efforts, regulatory enforcement, and an increase in funding.
