Abstract

Introducción: El Trastorno de Estrés Post-Traumático (TEPT) es una afección psiquiátrica que implica sufrir o presenciar un evento que ponga en riesgo la integridad personal. Su fisiopatología es primordialmente desconocida. Objetivo: Este estudio pretende clarificar su neurociencia más relevante para optimizar su comprensión y tratamiento. Como objetivo secundario, se aspira a manifestar la necesidad de un abordaje individualizado, con lo científico y humano como dos aspectos inseparables. Metodología: Se ha realizado una revisión sistemática según la metodología Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Como criterios de inclusión para la selección de artículos se ha considerado su origen, fecha de publicación y metodología científica. Como criterios de exclusión se han considerado casos clínicos concretos, diseños no corroborados científicamente, artículos sin texto completo y temáticas divergentes al tema. Así, se han obtenido 23 artículos de Pubmed, 2 de PsycINFO. Por añadidura, se ha obtenido información de 5 libros. Resultados y discusión: Destacan la afectación del Sistema Límbico, el Sistema de Recompensa Cerebral y el eje Hipotálamo-Hipofisario-Adrenal (eje HHA), además de neuroinflamación y cambios volumétricos encefálicos. Todo ello altera la extinción del miedo, la memoria contextual, y genera sintomatología intrusiva, evasiva, cognitiva, anímica y reactiva. El tratamiento se basa en la psicoterapia y farmacoterapia. Conclusiones: Se confirma el deterioro de las funciones cognitivas en los pacientes con TEPT, y se concluye que es una patología de afectación encefálica, psicológica y corporal. Se exponen posibles dianas terapéuticas más dirigidas que las presentes en la actualidad que faciliten futuras investigaciones.

Introduction: Post-Traumatic Stress Disorder (PTSD) is a psychiatric condition that involves experiencing or witnessing an event that puts personal integrity at risk. Its pathophysiology is primarily unknown. Objective: This study aims to clarify its most relevant neuroscience to optimize its understanding and treatment. A secondary objective is to highlight the need for an individualized approach, both scientific and humane as inseparable aspects. Methodology: A systematic review has been conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology. Inclusion criteria for article selection considered the origin, publication date, and scientific methodology. Exclusion criteria included specific clinical cases, unscientifically corroborated designs, articles without full text, and topics unrelated to the subject. A total of 23 articles from PubMed and 2 from PsycINFO were obtained. Additionally, information was gathered from 5 books. Results and discussion: The results highlight the involvement of the Limbic System, the Brain Reward System, and the Hypothalamic-Pituitary-Adrenal axis (HPA axis), as well as neuroinflammation and volumetric brain changes. All of this disrupts fear extinction, contextual memory, and generates intrusive, avoidant, cognitive, mood, and reactive symptoms. Treatment is based on psychotherapy and pharmacotherapy. Conclusions: Cognitive function impairment in PTSD patients is confirmed, and it is concluded that the condition affects the brain, psychological, and bodily systems. Possible therapeutic targets, more specific than those currently available, are presented to facilitate future research.
