La colangitis biliar primaria es una enfermedad hepática crónica y autoinmune de etiología desconocida. La mayor parte de los casos se diagnostican en mujeres, siendo una enfermedad poco frecuente. El pronóstico de esta sin tratamiento conlleva el desarrollo de cirrosis y la muerte, por lo que el diagnóstico precoz y la intervención farmacológica es fundamental en la historia de la enfermedad. El tratamiento que mejores resultados presenta hasta la fecha es el ácido ursodesoxicólico, sin embargo, hay un porcentaje amplio de pacientes que no responden al mismo. Para la evaluación de la respuesta se han establecido diferentes criterios que ayudan a determinar que pacientes son subsidiarios de recibir terapias de segunda línea, muchas de ellas aún en desarrollo. El ácido obeticólico es la alternativa aprobada para los no respondedores, aunque limitada a determinados pacientes. Los fibratos son otra opción muy prometedora en la actualidad, según los resultados de los ensayos clínicos que evalúan el uso de estos en la colangitis biliar primaria. Todas las alternativas terapéuticas son sinérgicas y se potencian al combinarse. El desarrollo de nuevas posibilidades terapéuticas está fomentando la elaboración de estudios internacionales con el fin de crear un consenso en el manejo de los pacientes más refractarios al tratamiento.
Primary biliary cholangitis is a chronic, autoimmune liver disease of unknown etiology. Most cases are diagnosed in women, making it a rare disease. Its prognosis without treatment entails the development of cirrhosis and death, so early diagnosis and pharmacological intervention is essential in the history of the disease. The treatment that has the best results to date is ursodeoxycholic acid; however, there is a large percentage of patients who do not respond to it. For the evaluation of the response, different criteria have been established that help determine which patients are eligible to receive second-line therapies, many of them still in development. Obeticholic acid is the approved alternative for non-responders, although limited to certain patients. Fibrates are another very promising option today, according to the results of clinical trials evaluating their use in primary biliary cholangitis. All therapeutic alternatives are synergistic and are enhance when combined. The development of new therapeutic possibilities is encouraging the development of international studies to create a consensus in the management of patients most refractory to treatment.
