Abstract

El momento ideal para que un paciente con ERC inicie la TRS, sigue siendo a día de hoy un aspecto que no está exactamente definido. En general, desde que el FG es inferior a 15 ml/min/1,73m2 debe considerarse la diálisis y cuando sea inferior a 6 ml/min/1,73m2, debe iniciarse la TRS independientemente de la clínica. Con el tiempo se ha visto que tanto el inicio temprano de la diálisis como su retraso, pueden implicar peor calidad de vida o peor pronóstico sobre el paciente. En este estudio observacional, se analizan las condiciones clínicas que tenían los pacientes del HUG antes de iniciar la TRS, observándose que la mayoría de los pacientes iniciaban la diálisis con un FG inferior a 9,5 ml/min/1,73 m2. Por otro lado, la nefropatía diabética se ha convertido en objeto de estudio en los últimos años ya que se trata de la causa más frecuente de entrada en diálisis. Se ha visto que en estos pacientes sería recomendable un inicio más precoz de la TRS, sin embargo, en nuestro estudio no se vieron diferencias en cuanto al FG con el que iniciaron la diálisis los pacientes con diabetes con respecto a los no diabéticos. Los factores que influyen a la hora de tomar una decisión sobre la TRS son múltiples y deben individualizarse en cada paciente, siendo imposible establecer un nivel de FG exacto que además también se verá condicionado por distintas variables.

The ideal time for a patient with CKD to initiate CRRT is still not exactly defined. In general, when GFR is less than 15 ml/min/1.73m2 dialysis should be considered and when it is less than 6 ml/min/1.73m2, RRT should be initiated regardless of the clinical condition. Over time it has been seen that both early initiation of dialysis and its delay, may imply worse quality of life or worse prognosis for the patient. In this observational study, the clinical conditions of HUG patients before starting RRT were analyzed, and it was observed that most patients started dialysis with a GFR lower than 9.5 mL/min/1.73 m2. On the other hand, diabetic nephropathy has become an object of study in recent years since it is the most frequent cause of dialysis initiation. It has been seen that in these patients an earlier initiation of RRT would be advisable; however, in our study there were no differences in the GFR at which patients with diabetes started dialysis compared to non-diabetic patients. The factors that influence the decision on RRT are multiple and must be individualized for each patient, and it is impossible to establish an exact GFR level, which will also be conditioned by different variables.
