Comorbilidades asociadas a la endometriosis

Abstract

La endometriosis es una enfermedad crónica de causa desconocida que puede afectar la calidad de vida severamente, llegando a ser incapacitante. La literatura científica ha asociado esta entidad con una mayor predisposición a desarrollar otras patologías, las cuales también contribuyen a aumentar la discapacidad. Recientemente, con el fin de esclarecer los mecanismos biológicos que desencadenan la endometriosis, se han propuesto diversas teorías en relación a dichas comorbilidades, al plantear que el riesgo aumentado de padecerlas puede deberse a alteraciones de factores o vías de señalización en común. En este trabajo se ha establecido la vinculación entre endometriosis y otras patologías que afectan a la salud. Para esto se ha utilizado la base de datos del año 2017 facilitada por la BDCAP de la Subdirección General de Información Sanitaria, calculando las prevalencias de problemas de salud por subpoblación. Los resultados refuerzan la extendida creencia, tanto en sociedades médicas como en asociaciones de pacientes, de que la endometriosis es una enfermedad infradiagnosticada. Por otro lado, este estudio no ha encontrado una asociación sólida entre endometriosis y desórdenes de la inmunidad o del aparato cardiovascular. En la esfera de la salud mental, se ha detectado un aumento de riesgo de padecer diversos trastornos psiquiátricos. Finalmente, proponemos una relación causal entre endometriosis y anemia de trastornos crónicos. Como conclusión, el personal sanitario debe considerar la endometriosis en su proceso de diagnóstico diferencial, y por tanto descartarla para minimizar el infradiagnóstico. Así mismo, es una patología crónica que afecta de manera importante la vida de las pacientes, causando dolor y sufrimiento.

Endometriosis is a chronic disease of unknown cause which may affect life quality in serious ways, even becoming incapacitating. Scientific literature has associated this entity with a greater predisposition to develop other pathologies, which also contributes to increase disability. Recently, in order to clarify the biological mechanisms that trigger endometriosis, several theories in relation to these comorbidities have been proposed. They argue that the increased risk may be due to disfunctions in common factors or signaling pathways. In this essay we’ve stabilished connections between endometriosis and other diseases that affect health. To achieve that goal the database of 2017 provided by the Primary Care Clinical Database (BDCAP) of the General Subdirection for Health Information has been used. We’ve calculated the prevalence of different medical conditions by subpopulation. The results reinforce the widespread belief, among medical societies and patient organisations, that endometriosis is an underdiagnosed condition. Additionally, this paper finds no relevant association between endometriosis and another inmunity-related or cardiovascular medical conditions. In the field of mental health it could be of particular interest the increased risk of various psychiatric disorders in pacients suffering from endometriosis. Finally, we propose a causal relationship between endometriosis and anemia of chronic disease. In conclusion, health workers should consider endometriosis in the differential diagnosis process, and so exclude it in order to minimize underdiagnosis. Besides, endometriosis is a chronic illness that badly affects the lives of patients, causing pain and suffering.
