Abstract

El azul de metileno (AM) es un fármaco utilizado en la medicina para el tratamiento de la metahemoglobinemia. Gracias a la mayor comprensión de su mecanismo de acción sobre la inhibición de la producción de óxido nítrico en las células musculares lisas, se han abierto nuevas líneas de investigación en situaciones críticas como los shocks distributivos. Cuando los shocks distributivos son refractarios a las primeras líneas de tratamiento, el AM tiene un papel esperanzador. Las primeras líneas en el shock son las catecolaminas, la fluidoterapia y la vasopresina. El azul de metileno actúa sobre una diana distinta, potenciándolas y realizando un efecto directo que se suma a la mejoría de la hemodinamia del paciente. Los estudios llevados a cabo son prometedores, aportando resultados estadísticamente significativos sobre el aumento de resistencias vasculares sistémicas y mejoras en la presión arterial. En este contexto inicial se debe profundizar y seguir investigando para desarrollar todos sus potenciales beneficios.

Methylene blue is a drug used in medicine for the treatment of methemoglobinemia. Thanks to the understand of its mechanism of action on the inhibition of the production of nitric oxide by smooth muscle cells, new lines of research have been opened in critical situations such as distributive shocks. When distributive shocks are refractory to the first lines of treatment, AM has a hopeful role. The first lines in shock are catecholamines, fluid therapy, and vasopressin. Methylene blue acts on a different target, enhancing them and improving patient's hemodynamics. The studies carried out are promising, providing statistically significant results on the increase of the sistemic vascular resistance, and improvements in blood pressure. In this initial context, it is necessary to deepen and continue researching to develop all its possible benefits.
