Desigualdades sociales en el acceso a los servicios sanitarios

Abstract

Introducción. El Sistema Nacional de Salud español tiene una vocación de cobertura universal. Sin embargo, pueden existir desigualdades sociales en el acceso de los servicios sanitarios como reflejo de los determinantes sociales de salud, creando un gradiente social en salud. El objetivo fue estudiar las desigualdades en el uso de los servicios sanitarios, el tipo de aseguramiento y las necesidades de atención sanitaria no cubiertas según la clase social y el país de origen. Metodología. Se incluyó la población de la Encuesta Europea de Salud en España de 2020 (N:21076). El uso de los servicios sanitarios fue analizado mediante las consultas reportadas al médico de familia, las consultas hospitalarias no urgentes, las hospitalizaciones y el uso de los servicios de emergencias. Las necesidades de atención sanitaria fueron medidas por falta reportada de asistencia por listas de espera y por problemas económicos. Los análisis estadísticos fueron realizados mediante regresiones logísticas ajustadas por sexo, edad y enfermedades crónicas. Resultados. Las clases sociales más bajas y la población extranjera presentaron mayor probabilidad de usar los servicios de atención primaria, emergencias y hospitalizaciones, así como de demandar mayores necesidades de atención sanitaria no cubiertas. Las clases sociales más altas y la población de origen español tuvieron mayor probabilidad de beneficiarse de un seguro médico privado. Conclusión. En nuestro país se produce una desigualdad en el acceso de los servicios sanitarios por clase social y país de origen, lo que puede repercutir en aumentar desigualdades importantes en salud. Deben implementarse políticas que las reduzcan.

Introduction. The Spanish National Health System has a vocation of universal coverage. However, social inequalities can exist in access to health services as a reflection of the social determinants of health, creating a social gradient in health. The main objective was to study the inequalities in the use of health services, the insurance type, and the unmet healthcare needs, according to social class and country of origin. Methodology. The population from the 2020 European Health Survey in Spain was included (N:21076). The use of health services was analyzed through reported consultations with the general doctor, non-emergency hospital consultations , hospitalizations, and the use of emergency services. Healthcare needs were measured by reported lack of attendance due to waiting lists and economic problems. Statistical analyzes were performed using logistic regressions adjusted for sex, age, and chronic diseases. Results. Lowe social classes and migrant population were more likely to use primary care, emergency, and hospitalization services, as well as to demand greater unmet healthcare needs. Higher social classes and natives were more likely to benefit from private health insurance. Conclusion. There is inequality access to health services by social class and country of origin in Spain, which can have repercussions in increasing health inequalities. Policies must be implemented to reduce them.
