Abstract

INTRODUCCIÓN: El Síndrome de ovario poliquístico es una patología heterogénea que tiene como base el hiperandrogenismo. La instauración de este exceso androgénico en las diferentes etapas del desarrollo fetal, infantil o adolescente supone un factor de susceptibilidad a desarrollar esta patología. La asociación del hiperandrogenismo con otros factores tales como el hiperinsulinismo, la resistencia a la insulina y la obesidad, hacen que el diagnóstico del síndrome de ovario poliquístico sea complejo durante la adolescencia. MÉTODOS: Con el fin de llevar a cabo el desarrollo de este trabajo, se ha revisado la bibliografía disponible con los estudios más relevantes sobre hiperandrogenismo, las diferentes perspectivas fitopatológicas y diagnósticas de esta patología, y su relación con la aparición del hiperandrogenismo en diferentes edades. RESULTADOS: El hiperandrogenismo es el hilo conductor del síndrome de ovario poliquístico y tiene diferentes presentaciones dependiendo de la etapa del desarrollo en la que se manifieste. CONCLUSIONES: El síndrome de ovario poliquístico supone un reto diagnóstico debido a la deficiencia de criterios estandarizados en la edad adolescente. El hiperandrogenismo ha de ser sospechado durante esta edad ante la presencia de signos tan característicos como el hirsutismo, las irregularidades menstruales, un acné grave o los antecedentes personales de una adrenarquia exagerada. Esta dificultad diagnostica, también repercute en la dificultad de instaurar un tratamiento precoz que disminuya el riesgo de presentación de las alteraciones metabólicas asociadas más tarde a infertilidad y trastornos que influirán en la salud y el bienestar psicológico de estas mujeres en la edad adulta.

BACKGROUND: Polycystic ovary syndrome is a heterogeneous pathology based on hyperandrogenism. The establishment of this androgenic excess in the different stages of fetal, infantile or adolescent development supposes a susceptibility factor to develop this pathology. The association of hyperandrogenism with other factors such as hyperinsulinism, insulin resistance and obesity, make the diagnosis of polycystic ovary syndrome a complex pathology during adolescence. METHODS: In order to carry out the development of this work, we have reviewed the literature available with the most relevant studies on hyperandrogenism, the different phytopathological and diagnostic perspectives of this pathology, and its relationship with the onset of hyperandrogenism at different ages. RESULTS: Hyperandrogenism is the common thread of polycystic ovary syndrome and has different presentations depending on the stage of development in which it manifests. CONCLUSIONS: Polycystic ovarian syndrome is a diagnostic challenge due to the deficiency of standardized criteria in adolescent age. Hyperandrogenism should be suspected during this age in the presence of such characteristic signs as hirsutism, menstrual irregularities, severe acne or the personal history of exaggerated adrenarche. This diagnostic difficulty also affects the difficulty of establishing early treatment that reduces the risk of presenting the metabolic disorders later associated with infertility and disorders that will influence the health and psychological well-being of these women in adulthood.
