Caracterización hemodinámica doppler e inflamatoria de las preeclámpticas: análisis del efecto del tratamiento con parches de nitroglicerina

Abstract

De esta investigación prospectiva de la caracterización hemodinámica Doppler e inflamatoria de pacientes con preeclampsia y de un grupo control de embarazadas normotensas sanas, así como el análisis del efecto del tratamiento con parches de nitroglicerina en las preeclámpticas, se concluye que: Las preeclámpticas tiene valores significativamente más latos de velocimetría Doppler en las arterias uterinas, umbilical, renal fetal y hepática fetal y más bajos de la arteria cerebral media fetal comparado con las embarazadas normotensas. Las pacientes con diagnóstico de preeclampsia presentaron concentraciones más altas de los marcadores de inflamación interuquina-6, factor de necrosis tumoral alfa, interferón gamma y proteína C reactiva y de los marcadores de disfunción endotelial selectina P y E. Sin embargo, las concentraciones de interleuquina-10, selectina L, nitrato / nitrito y sustancia reactivas al ácido barbitúrico fueron más bajas en este grupo de pacientes al compararlo con los controles. El uso de parches de nitroglicerina produce reducción significativa en el índice de pulsatilidad y resistencia y en la relación sistólica/diastólica de las arterias uterinas, sin producir modificaciones significativas en los índices de velocimetría doppler de la arteria umbilical, arteria cerbral media fetal, arteria renal fetal y arteria hepática fetal. El tratamiento con parches de nitroglicerina no produce modificaciones en las concentraciones séricas de indicadores de inflamación y disfunción endotelial. Sin embargo, se observó aumento en las concentraciones plasmáticas de nitrato /nitrito y sustancia reactivas al ácido barbitúrico. El uso de parches de nitroglicerina no modifica en forma significativa las variables neonatales, el desarrollo de complicaciones perinatales y la mortalidad perinatal. Sin embargo, existe una reducción en la incidencia de complicaciones y la mortalidad perinatal en las preeclámpticas tratados. El empleo de los parches de nitroglicerina se asoció a una mayor frecuencia de cefalea.
