Velocidad de onda de pulso y onda de reflexión precoz en el embarazo con riesgo de preeclampsia

Abstract

Objetivo La preeclampsia (PE) es una enfermedad de etiología desconocida cuyos predictores diagnósticos son tardíos . El objetivo del presente trabajo es detectar si existe una alteración precoz de la distensibilidad arterial, evaluada por velocidad de onda de pulso (VOP) y onda de reflexión precoz (ORP), en embarazadas con alto riesgo de PE . Material y métodoSe incluyeron 54 pacientes que cursaban 20 a 30 semanas de gestación, 27 sin antecedentes (G1), y 27 con PE, hipertensión gestacional o crónica en gestaciones previas (G2). Por tonometría con SphygmoCor se obtuvieron: onda de pulso en arteria radial y carótida y por función de transferencia se derivó la onda de la aorta ascendente, analizando en ellas las presiones sistólica y diastólica, la VOP y la ORP. Los datos se analizaron con las pruebas de la t de Student y U de Mann-Whitney con un valor de p con significación estadística < 0,05 y las correlaciones con r de Pearson y rS de Spearman . ResultadosLas medias y desvíos estándar en G1/G2 fueron : edad : 25,2 ± 6,5 129,5 ± 6,5 (p < 0,015) ; presión arterial sistólica (PAS) radial : 108 ± 9 1122 ± 13 (p < 0,001) ; presión arterial diastólica (PAD) radial : 63 ± 9,3 / 73 ± 11 (p < 0,001) ; PAS aórtica: 93 ± 8,2 / 107 ± 16 (p < 0,008) ; PAD aórtica: 65 ±9,5 / 74 ± 10 (p < 0,001); ORP: 1,6 ± 4 / 4,6 ± 6,7 (p > 0,09) ; VOP: 6,8 ± 1,6 17,9 ± 1,8 (p < 0,035) . Conclusión La distensibilidad arterial en el embarazo con alto riesgo de PE está alterada precozmente evaluada a través de la VOP
