Feocromocitoma con catecolaminas reiteradamente normales

Abstract

El feocromocitoma como causa de hipertensión arterial secundaria es una patología infrecuente y su presentación con catecolaminas y sus metabolitos normales es aun más rara. Dado el comportamiento paroxístico de la liberación de estas sustancias, ante cuadros de alta presunción clínica es importante reiterar las determinaciones, especialmente con dosajes intracrisis. Se presenta el caso de una paciente de sexo femenino que permaneció 10 años sin diagnóstico etiológico, con dosajes reiterados y estudios por imágenes sin evidencia de patología, hasta que una ecografía abdominal demostró el agrandamiento de la glándula suprarrenal derecha, que posteriormente se confirmó por TAC abdominal y centellograma radioisotópico con yodometilbencilguanidina (IMBG). Se realizó una adrenalectomía derecha sin complicaciones y se confirmó el diagnóstico histológico de feocromocitoma.
