Angina inestable

Abstract

Nuestro grupo, considerando la forma de presentación clínica, los antecedentes anginosos, los distintos cambios ecocardiograficos y la respuesta al tratamiento, definió cada presentación clínica con sus diferentes pronósticos. Pacientes con infradesnivel del segmento ST tienen mayor sobrevida que los que presentan infradesnivel del segmento ST con dolor (82% versus 70%). El angor inestable y supradesnivel del segmento ST comprenden a pacientes muy jóvenes, con angina de reciente comienzo, como forma de presentación clínica mas frecuente y con mayor riesgo de infarto agudo de miocardio intra-hospitalario, con escaso riesgo y mortalidad en el seguimiento. El infradesnivel del segmento ST abarca al grupo de mayor edad, con angina progresiva como forma de presentación mas común, con mayores antecedentes de angina crónica estable e infarto agudo de miocardio y mas elevada tasa de mortalidad intra-hospitalaria y a largo plazo.
