Abstract

La existencia de hipertrofia ventricular izquierda ha sido identificada como un poderoso factor de riesgo para la aparición de muerte súbita y otras patologías cardiovasculares. La hipertrofia ventricular izquierda da origen a extrasistolia ventricular incluso en ausencia de enfermedad de las arterias coronarias. Los mecanismos de generación de las extrasístoles son multifactoriales e involucran el agrandamiento de la fibra miocárdica y la hipertrofia medial de las arterias coronarias, así como también la presencia de áreas focales de fibrosis y de isquemia subendocárdica que puedan impedir la propagación normal del impulso a través del miocardio. La hipertrofia ventricular izquierda puede ser reducida por terapéutica antihipertensiva específica, pero no todas las drogas son igl!almente potentes en este aspecto. Se ha demostrado una reducción de la masa ventricular durante el tratamiento con drogas antiadrenérgicas de acción central, inhibidores de la convertasa, antagonistas del calcio, betabloqueantes y algunos bloqueantes alfa, pero no con hidralazina, minoxidil y sólo en menor grado usando diuréticos. EI tratamiento con bloqueantes del calcio y con betabloqueantes permite esperar no sólo una disminución de la masa ventricular sino también de las arritmias ventriculares. Queda por definir si esta reducción de la hipertrofia ventricular izquierda y la consiguiente supresión de la ectopía ventricular mejoran el pronóstico desfavorable que tienen estos pacientes.
