Polimorfismos COL2A1 y GDF5 en el tratamiento en pacientes con osteoartritis de rodilla.

Abstract

La osteoartritis de rodilla es una patología degenerativa articular con gran prevalencia, caracterizada por la pérdida progresiva de cartílago, la presencia de alteraciones del hueso subcondral y por la limitación funcional de los pacientes, cuyo curso clínico se ve modificado por factores mecánicos, metabólicos, inflamatorios y/o factores genéticos; el presente estudio descriptivo y analítico se realizó en base a una revisión reciente de la literatura en bases de datos internacionales, priorizando un total de 34 referencias correspondientes a los últimos cinco años así como 10 estudios principales que fueron seleccionados a partir de una serie de criterios muy rigurosos; la evidencia descrita muestra que los polimorfismos en COL2A1, el gen responsable de codificar el colágeno tipo II, debilitan la matriz cartilaginosa y facilitan el desgaste, mientras que las variantes en GDF5, un gen que debe regular la proliferación y diferenciación de condrocitos, acentúan la reducción de la capacidad regenerativa del tejido y de la evolución clínica; así mismo se ha mostrado que, a pesar de tener estudios radiológicos con hallazgos similares, los pacientes muestran respuestas muy distintas a terapias tipo fisioterapia, infiltraciones de ácido hialurónico o PRP, lo que demuestra que el perfil genético también determina la respuesta a la terapia; se puede concluir que la identificación de polimorfismos en COL2A1 y GDF5 es un hallazgo clave en el sentido de aportar la genética a la medicina personalizada, que permite estratificar y anticipar pronósticos y tomar decisiones clínicas más precisas.
