Tratamento da Valva Mitral por Cateter: Reparo ou Substituição?

Abstract

A regurgitação mitral (RM) é uma condição prevalente, especialmente em idosos, e está associada a significativa morbimortalidade, especialmente em pacientes com comorbidades e alto risco cirúrgico. O tratamento convencional, o reparo cirúrgico da válvula mitral (VM), é eficaz, mas muitos pacientes não são candidatos devido ao risco cirúrgico elevado. Como alternativa, o reparo e a substituição transcateter da válvula mitral têm emergido como opções promissoras, com destaque para a técnica de reparo do folheto de ponta a ponta, aprovada pela FDA, e a substituição da válvula por cateter (TMVR), especialmente para casos com anatomia desfavorável ou válvulas biopróteses degeneradas. A seleção adequada de pacientes é crucial e envolve o uso de técnicas de imagem, como a tomografia computadorizada (TC), para avaliar a anatomia valvar e prever complicações. Apesar dos avanços, o desenvolvimento de dispositivos transcateter específicos para a posição mitral ainda é limitado, com poucos dispositivos comercializados e outros em fase de avaliação clínica. Este estudo revisa o estado atual das técnicas de reparo e substituição transcateter da VM, explorando suas aplicações clínicas, resultados, limitações e perspectivas futuras.
