Abstract

Introducción: El pie diabético infeccioso (PDI) es una entidad de alta morbimortalidad, especialmente cuando progresa a infección profunda con compromiso compartimental. La diseminación infecciosa puede aumentar la presión compartimental, comprometer la perfusión y favorecer necrosis y fascitis. En este contexto, el diagnóstico de síndrome compartimental suele ser clínico y requiere intervención quirúrgica oportuna. La fasciotomía temprana, asociada a terapia de presión negativa (TPN), podría contribuir al control del proceso infeccioso y a la cicatrización.Objetivo: Describir los resultados clínicos de la fasciotomía amplia mediante abordaje tipo “palo de hockey”, asociada a TPN, en pacientes con PDI y síndrome de respuesta inflamatoria sistémica (SRIS). Método: Estudio retrospectivo descriptivo que incluyó pacientes con PDI, alta sospecha clínica de compromiso compartimental y SRIS sin disfunción orgánica. El diagnóstico se estableció clínicamente. Se realizó fasciotomía amplia y TPN con sistema Renasys® EZ Max a –125 mmHg en modo continuo. Los pacientes fueron egresados con criterios clínicos de estabilidad y seguimiento ambulatorio estrecho.Resultados: Se incluyeron tres pacientes masculinos con diabetes mellitus tipo 2 de larga evolución. Todos presentaron infección del pie con SRIS sin disfunción orgánica. No se registraron complicaciones ni reingresos. La estancia hospitalaria fue de 24 a 48 horas. Dos pacientes requirieron amputación digital limitada. El tiempo de cicatrización fue de 7 a 13 semanas.Conclusión: La fasciotomía amplia asociada a TPN podría facilitar el control temprano de la infección y permitir manejo ambulatorio precoz en pacientes seleccionados. Se requieren estudios comparativos con mayor tamaño muestral.
