Bradiarritmia, um achado raro na infeção por Legionella pneumophila

Abstract

A infeção por Legionella pneumophila (LP) provoca, na maior parte das vezes, uma doença sistémica. O envolvimento cardíaco é raro, contudo, pode apresentar- -se na forma de miocardite, pericardite, endocardite, derrame pericárdico e mais raramente, como perturbação do ritmo cardíaco. Revendo a literatura disponível estão descritos quatro casos de perturbação da condução cardíaca, sendo este o primeiro caso descrito de disfunção binodal. Os autores apresentam o caso de um homem de cinquenta e cinco anos de idade, admitido numa unidade de cuidados intensivos por pneumonia a LP com evolução para choque séptico e síndrome de dificuldade respiratória aguda grave, a motivar início de técnica de oxigenação por membrana extracorporal veno-venosa. Durante o período de infeção ativa, sob antibioterapia com azitromicina, foram documentados episódios de pausas sinusais e bloqueio auriculoventricular completo, associados a instabilidade hemodinâmica, com necessidade de implantação de pacemaker provisório. Após resolução do quadro infecioso, houve resolução completa das alterações do ritmo cardíaco, sem recorrência durante os seis meses de seguimento após a alta hospitalar. A infeção por LP é uma causa rara mas reversível de bradiarritmia, sendo a evolução favorável com a resolução do quadro infecioso sistémico.
