Abstract

La aspergilosis pulmonar invasiva (API), una infección fúngica de alta mortalidad en inmunocomprometidos, representa un desafío diagnóstico. Se presenta el caso de un hombre de 68 años con hipertensión, enfermedad renal crónica e hipertiroidismo, quien no desarrolló API con neutropenia, sino con linfopenia severa de células T CD4+ (162 cel/μl). Sus síntomas crónicos e inespecíficos (tos productiva, disnea, pérdida de peso por cuatro años, sin fiebre) contrastaron con la clínica típica, llevando a valoraciones previas erróneas. El diagnóstico se confirmó mediante hallazgos compatibles en la tomografía computarizada (TC) de tórax, visualización de hifas septadas, cultivo positivo para Aspergillus fumigatus en esputo y positividad del galactomanano sérico, lo cual permitió una evaluación probable según los criterios de la Organización Europea para la Investigación y el Tratamiento del Cáncer y el Consorcio de Educación e Investigación del Grupo de Estudio de Micosis (EORTC/MSGERC) de 2020. A pesar de no ser el tratamiento de primera línea, el itraconazol (200 mg c/12h) indujo una notable mejoría clínica, radiológica y microbiológica. Este caso subraya que la linfopenia T CD4+ severa es un factor de riesgo emergente para API en pacientes no neutropénicos. Una alta sospecha clínica, un diagnóstico multidisciplinario temprano y un tratamiento antifúngico oportuno son cruciales para mejorar el pronóstico en esta población.
