Intubación vigil con fibrobroncoscopio flexible en mixoma odontogénico extenso: asegurando la vía aérea

Abstract

Introducción: La vía aérea difícil secundaria a tumores oromaxilofaciales constituye un reto anestésico considerable, donde el uso de fibrobroncoscopio o video endoscopio bajo sedación consciente es el estándar de referencia para preservar la vía aérea. Caso clínico: Mujer con tumoración indurada en maxilar derecho posterior a exodoncia, programada para resección quirúrgica extensa y reconstrucción facial. Se realizó sedación consciente con remifentanilo en infusión y topicalización de la vía aérea con lidocaína al 10% (4 pufs). Posteriormente, se efectuó intubación fibrobroncoscópica con la paciente despierta, visualizando la lesión maxilar y las cuerdas vocales, a través de las cuales se introdujo un tubo endotraqueal anillado N° 7. La paciente toleró el procedimiento y obedeció órdenes durante la inserción. La anestesia se indujo con propofol y rocuronio, y se mantuvo con remifentanilo y sevofluorane. Intervención y resultado: La técnica permitió una intubación segura y controlada, evitando complicaciones respiratorias y facilitando el abordaje quirúrgico. Conclusiones: El fibrobroncoscopio flexible es una herramienta versátil y segura para el manejo de la vía aérea difícil en pacientes despiertos con tumores oromaxilofaciales. Este caso resalta su utilidad en escenarios con alteración anatómica significativa.
